Clinical Observation Of Zhang's Add And Subtract Peach Kernel Soup Combined With Lower Limb Fumigation Drug In The Treatment Of Ischemic Necrosis Of Adult Femoral Head With Qi And Blood Stasis Type

Objective:To observe the short-term clinical effect of zhang's add and subtract peach kernel soup combined with lower limb fumigation drug in the treatment of ischemic necrosis of adult femoral head with qi and blood stasis type.Methods:Sixty patients with osteonecrosis of femoral head who met the criteria for case selection from January 2018 to September 2018 in the orthopaedics and traumatology department of zhangzhou hospital of traditional Chinese medicine were collected.Prospective clinical trials were used to study 60 cases of adult osteonecrosis of femoral head All cases were treated with zhang's add and subtract peach kernel soup combined with lower limb fumigation drug.Oral addition or reduction of taoren decoction and lower limb fumigation drug were given once a day in the morning and once in the evening,with 6 weeks as a course of treatment,a total of 2courses of treatment,as well as routine health education and bed rest,so as to reduce the load activity of hip joint.Observation points were set at before treatment,treat 6 weeks,treat 12 weeks and 3 months after the end of treatment.Harris hip function score,VAS score and TCM syndrome quantitative score were evaluated before treatment,6 weeks after treatment,12 weeks after treatment,and 3 months after the end of all treatment courses,radiographically,the effective rate of follow-up was compared by X-ray before treatment,treat 12 weeks,and3 months after treatment,and the range of necrosis index was compared by MRI before treatment and 3 months after all treatments.SPSS 20.0 statistical software was used for statistical analysis of the data collected.Results:(1)The Harris scores of the affected hip before treatment were 54.24±4.32,and the Harris scores of the affected hip at treat 6 weeks,treat 12 weeks,3 months after treatment were63.09±4.52,66.20±5.09 and 67.8±4.27,respectively,the differences were statistically significant(P<0.01);(2)The VAS scores before treatment were 5.15±0.87,and the VAS scores at treat 6 weeks,treat 12 weeks,3 months after treatment were 3.12±0.77,2.38±0.55,and 1.95±0.89,respectively,the differences were statistically significant(P<0.01);(3)Scores of TCM syndromes before treatment were 12.13±1.09,and scores of TCM syndromes at treat6 weeks,treat 12 weeks,3 months after treatment were 14.82±1.07,15.58±1.01 and15.93±0.90,respectively,the differences were statistically significant(P<0.01);(4)The effective rate of X-ray reexamination after 12 weeks and 3 months of radiological treatment was 98.18% and 92.73%,respectively,and the difference was not statistically significant(P>0.05);The follow-up MRI before treatment and after 3 months showed that the femoral head necrosis index was 21.22±8.30 and 20.97±8.06,respectively,and the difference was statistically significant(P< 0.05).Conclusion:In the treatment of adult femoral head ischemic necrosis with qi and blood stasis type,zhang's add and subtract peach kernel soup combined with lower limb fumigation drug has a relatively satisfactory short-term clinical effect and few side effects in improving the function of the patient's hip joint,alleviating the patient's pain,improving symptoms of traditional Chinese medicine and preventing further collapse of the femoral head,it is a safe andeffective scheme with the characteristics of traditional Chinese medicine to treat adult femoral head ischemic necrosis in the early and middle stage,which is worthy of clinical application.
