Objective:To investigate the characteristics of the patients with acute coronary syndrome of the coronary artery non-total occlusion.Methods:According to the degree of of coronary artery occlusion,based on the results of angiography and exclusion criteria,patients diagnosed with ACS were divided into two groups:?1?total occlusion group;?2?non-total occlusion group.Statistical analysis was performed by collecting baseline data,laboratory tests,coronary imaging,and in-hospital adverse events from patients.Results:A total 592 ACS patients including 303 cases?51.18%?with total occlusion and289 cases?48.82%?with non-total occlusion were analyzed.Of those,66 cases?11.15%?with non-atherosclerotic ACS were excluded.The baseline data The time of chest pain 10?6,22?h,systolic blood pressure?128.15±25.02?mmHg,40 cases?17.93%?with dyslipidemia,214 cases?95.96%?with aspirin,214 cases?95.96%?with P2Y122 inhibitor,216 cases?96.86%?with statins and 165 cases?73.99%?with beta blockers in non-total occlusion group were higher than the time of chest pain 7?4,12?h,systolic blood pressure?122.06±23.5?mmHg,33 cases?10.89%?with dyslipidemia,263 cases?86.80%?with aspirin,263 cases?86.80%?with P2Y122 inhibitors,273 cases?90.10%?with statins and 191cases?63.03%?with beta blockers in total occlusion group,P<0.05;Laboratory test Mb118?40,406?ng/L in non-total occlusion group was lower than 363?107,742?ng/L in total occlusion group,P<0.05;EF?50.68±11.05?%in non-total occlusion was higher than EF?46.98±11.12?%in total occlusion group,P<0.05;Imaging The degree of occlusion?96.21±4.15?%,RCA lesions had 42 cases?18.83%?,PTCA had 4 cases?1.8%?,thrombus aspiration had 13 cases?5.83%?,tirofiban use had 134 cases?60.09%?and stent implantation diameter in non-total occlusion group were lower than the degree of occlusion?100.00±0.00?%,115 cases?37.95%?with RCA lesion,21 cases?6.93%?in PTCA,64cases?21.12%?in thrombus aspiration,239 cases?78.87%?with tirofiban and stent diameter?3.16±0.45?mm in total occlusion group,P<0.05,while TIMI blood flow?2.49±0.82?grade and 138 cases?61.88%?at LAD lesion in non-total occlusion were higher than TIMI?0.00±0.00?grade and the lesions of 136 cases?44.88%?at LAD in total occlusion,P<0.05.Adverse events All-cause death?0.00?%,re-infarction?0.00?%,heart failure had 9 cases?4.03%?,9?4.03%?cardiogenic shock and 36 cases?16.14%?with all-cause events in non-total occlusion group were lower than all-cause death were 15cases?4.95%?,9 cases?2.97%?with secondary myocardial infarction,36?11.88%?heart failure,27?8.91%?cardiogenic shock and 96 cases?31.68%?with all-cause events in total occlusion group,P<0.05,while transfusion had 4 cases?1.79%?in non-total occlusion group was higher than transfusion 0.00%in total occlusion group,P<0.05.Conclusion:Compared with ACS with total occlusion coronary artery,ACS with non-total occlusion of coronary artery has longer chest pain,higher use of aspirin,P2Y12inhibitors,statins and beta blockers,more stable hemodynamics,more common vascular lesions with LAD,fewer adverse events in hospital and better prognosis.According to the characteristics of atherosclerotic ACS with with non-total occlusion of coronary,early diagnosis and intervention may be made to reduce adverse events and improve the prognosis of ACS with non-total occlusion of coronary.
