Objective : To explore whether the use of endotracheal tube wall cleaning can effectively reduce the incidence of ventilator associated pneumonia.Method: Using prospective,randomized,single blind design,children who received mechanical ventilation for more than 48 hours in the department of critical care medicine of the children's hospital of Chongqing medical university during the period January 2016 to January 2017 were randomly divided into three groups by random number table,each group consists of 108 cases.Three groups were named :the control group,experimental group 1(the inner wall of the tracheal catheter removal once a day),experimental group 2(the inner wall of the tracheal catheter removal twice a day).Reference to the 2013 edition of the Chinese Critical Care Medicine published in the clinical diagnosis of ventilator associated pneumonia.Statistical analysis the ventilator associated pneumonia incidence,endotracheal tube plugging rate,mechanical ventilation time,PICU stay time and hospitalization time.Results: A total of 324 patients were included in the study,There were 35 cases of ventilator-associated pneumonia in the control group.There were 24 cases of ventilator-associated pneumonia in the experimental group 1,while there were 13 cases of ventilator-associated pneumonia in the experimental group 2.There was significant difference between the 3 groups(P<0.05).The occurrence of plugging condition: in the control group,9 cases were blocked,no blockage occurred in the experimental groups.There was significant difference between the 3 groups(P<0.01).However,there was no significant difference between the groups in mechanical ventilation time,PICU time and hospitalization time(P>0.05).Conclusion: Endotracheal tube wall cleaning can effectively reduce the incidence of ventilator-associated pneumonia,and significantly reduce the incidence of tracheal catheter blockage.
