Objective: To analyze the clinical features and therapeutic effects of 100 cases patients with middle ear cholesteatoma,which is providing basis and references for further improving the therapeutic effect.Materials and methods: Analyzing and collecting 100 cases patients with middle ear cholesteatoma who underwent cholesteatoma surgery in Shengjing Hospital Affiliated to China Medical University from December 2015 to April 2017 and followed up for more than 6 months.Analyzing the clinical classification,course of disease,complications,preoperative hearing loss,the relationship between the type of cholesteatoma and hearing impairment,the operative methods,the time of postoperative dry ear,the relationship between operation and the time of dry ear.Designing and analyzing the data by spss software.Results: There were 44 cases of upper tympanic type,53 cases of invagination and adhesion,and 2 cases of secondary operation in 100 cases of middle ear cholesteat,.The main symptoms of middle ear cholesteatoma were hearing loss and ear discharge.The average duration of ear discharge was 16.06±14.79 years.There were 100 cases of hearing loss,and the duration of hearing loss was 15.80±14.07 years.There were 46 cases of conductive deafness(46%),44 cases of mixed deafness(44%),10 patients had neurosensory deafness with severe and severe sense of hearing(10%).The degree of hearing loss was closely related to the clinical classification of middle ear cholesteatoma(P <0.05).The complications of cholesteatoma in the middle ear were labyrinthitis,meningeal irritation,and facial paralysis.The incidences were labyrinthitis in 14 cases(14%),meningeal irritation in 7 cases(7%),facial paralysis in 2 cases(2%)..The surgical procedure was based on mastoid surgery combined with middle ear surgery plus external auditory canal surgery;the average dry ear time was 7.76±3.39 years.The surgical approach and clinical classification of cholesteatoma were closely related to dry ear time(P< 0.05).Conclusion: The main clinical manifestations of cholesteatoma in the middle ear are hearing loss and ear pus;the course of the disease is more than ten years;complications such as labyrinthitis,meningeal irritation and facial paralysis;surgical treatment is effective,and the surgical procedure is better chosen to mastoid Surgery combined with tympanic surgery plus external auditory canal surgery;ossicular chain reconstruction has a good effect of hearing improvement,80% of patients consciously improve their hearing;the average dry ear time after middle ear cholesteatoma is about one and a half months;The classification of cholesteatoma in the middle ear is clinically significant.Both upper tympanic cholesteatoma and invaginated cholesteatoma of the middle ear are associated with hearing loss,complications,postoperative hearing improvement,and postoperative dry ear time.Are different,can be used as a surgical choice and an important reference to determine the prognosis.
