| Objective: To investigate the clinical effect of anterior cervical 2-3 disc resection and intervertebral bone graft fusion and internal fixation in the treatment of Hangman fracture complicated with disc injury.Methods: From December 2005 to December 2016,15 patients with Hangman fracture treated in our hospital were treated with anterior cervical 2-3 disc resection,intervertebral fusion plate and screw fixation,including 10 males and 5 females.The average age was 39.5 ±10.5 years.According to Levine-Edwards classification,7 cases of fracture were type Ⅱ,4 cases of type Ⅱ A and 4 cases of type Ⅲ.The standard ASIA score was used to evaluate the neurological function of the patients.The fracture healing was evaluated by the results of X-ray and CT of the cervical vertebrae.The operative complications were recorded during and after operation.Results: All the 15 patients were followed up for an average of 17 ±10 months.All patients underwent anterior cervical 2-3 disc resection and intervertebral bone fusion and internal fixation.The NDI scores of preoperative,1 week and last follow-up were 41.36±2.14、14.59±2.18、14.45±1.76,respectively.The JOA score was 5.12±1.69、11.23±2.17、12.25±2.71,respectively.The VAS score was 7.15±2.31、2.13±1.18、1.45±2.01 respectively.The ASIA score was improved from grade C to grade D in 1 case and from grade D to grade E in 4 cases.During the last follow-up,there were no complications such as fracture displacement,loosening of internal fixation,incision infection,submandibular gland leakage and goose neck malformation.The imaging results of the last follow-up showed that 2-3 segments of the neck were bony healing.Conclusion: Anterior cervical intervertebral disc resection and intervertebral fusion and internal fixation for Hangman fracture with cervical 2-3 disc injury. |
