Relationship Between Neutrophil-lymphocyte Ratio、peripheral Blood Platelet-lymphocyte Ratio And Arteriovenous Fistula Stenosis In Maintenance Hemodialysis Patients

Objective: To get the relevance among the neutrophil lymphocyte ratio(NLR),the platelet lymphocyte ratio(PLR)and the arteriovenous fistula stenosis of maintenance hemodialysis patients,so as to provide a convenient way for clinical prediction of the arteriovenous fistula stenosis.Method: Select 307 cases of admitted ESRD patients between January 2010 and February 2016 that will be treated with arteriovenous fistula plasty or recanalization,and 50 healthy cases as the control group.Divide the them into the stenosis group and the non-stenosis group depending on whether they belong to the arteriovenous fistula stenosis or not,and then calculate the NLR and PLR based on the history taking and blood biochemical test to study the correlation among NLR,PLR and the arteriovenous fistula stenosis.At the same time,collect venous segments of 18 patients to observe the morphology 、 measure the intima thickness of the segments.Study the relationship among NLR,PLR and vein intima-media thickness.Results:(1)The white blood cell count,neutrophil count,NLR and PLR in stenosis group were significantly higher than those in N group and non-stenosis group(P<0.01),while the lymphocyte count in stenosis group was lower than that in other groups(P<0.01).Compared with N group,the neutrophil count and NLR increased(P<0.01),lymphocyte count and platelet count decreased(P<0.01)in nonstenosis group,and their PLR difference has no statistical significance(P>0.05).Pearson correlation analysis indicated that the NLR and PLR were positively correlated with the white blood cell count and LDL-C in stenosis group.Multivariate logistic regression analysis showed that the white blood cell count andNLR were risk factors for arteriovenous fistula stenosis.Self-comparison T-test revealed that the differences in neutrophil count,lymphocyte count,and NLR were statistically significant(P<0.05).However,there was no significant difference in the platelet count and PLR(P>0.05).(2)The average intimal thickness of the vein segments was positively correlated with RBC,NLR,Hb,TG,P,Mg,while the average medial thickness was positively correlated with P.Multiple stepwise regression analysis displayed that NLR and P could influence average intimal thickness.Conclusion: For patients with maintenance hemodialysis,NLR and PLR can be taken as the predictors of the arteriovenous fistula stenosis.
