Objective: Pregnancy is a complex physiological process that produces changes in cytokines during pregnancy and childbirth.These changes are important for pregnancy maintenance.Recurrent spontaneous abortion(RSA)is defined as two or more consecutive abortions before 20 weeks of gestation and is a pregnancy complication of approximately 3%of fertile couples [1].Although RSA is considered to be a risk factor for its occurrence,most patients with recurrent spontaneous abortion are associated with immunomodulatory abnormalities.Considering the immunomodulatory properties of vitamin D(VD),in addition to its key role in calcium homeostasis,it has recently been found that deficiency or deficiency of vitamin D may directly affect pregnant women and is associated with specific pregnancy outcomes,such as recurrence.Sexual abortion,pre-eclampsia and gestational diabetes.We speculate that patients with recurrent spontaneous abortion may be associated with vitamin D levels,and vitamin D may be a potential link between the immune mechanisms that explain recurrent abortion.Methods :Seventy-seven patients with RSA who were admitted to our hospital from December 2008 to December 1818 were selected as the case group.The number of spontaneous abortions was at least 2,6-18 weeks of gestational age,and the age ranged from20 to 35 years old.Enrollment criteria: 1 chromosomal karyotypes of both husband and wife were normal;2 color ultrasound excluded genital tract lesions;3 cervical secretion examination excluded genital tract infection;4 reproductive endocrine hormone test no abnormalities,and no history of endocrine disease;5 most recent 3 No endocrine or immunomodulatory agents and vitamin D were used within the month;6 men's semen routine examination showed no abnormalities.In the same period,68 women with no history of adverse pregnancy were included,and women with at least one normal birth history were included as normal controls.There were no significant differences in age,menstrual cycle,gestational age,and body mass index(BMI)between the two groups(P>0.05).Both groups of subjects received 2 ml of fasting venous blood in the morning to collect blood.Then,using a double-antibody sandwich enzyme-linked immunosorbent assay(ELISA),a standard curve was drawn to calculate 25-OH-VD,interleukin-6(IL-6),and leukocyte-mediated Inter-10(interleukin-10,IL-10),tumor necrosis factor-?(tumor necrosis factor,TNF-?)concentration.Take the standard concentration as the abscissa,the corresponding OD value as the ordinate,use EXCEL to draw the scatter plot,then add the regression curve(straight line),and obtain the regression equation and the square of the regression coefficient.The grouped data were analyzed by SAS9.2 software.The measurement data were expressed by mean ħmark difference(xħs).The comparison between groups was performed by unequal sample analysis of variance(GLM).The mean value of measurement data was analyzed by F test,p<0.05.The difference was significant,and p < 0.01 indicates that the difference was extremely significant.The correlation between the control group and the case group and25-OH-VD,IL-6,IL-10,TNF-? was compared to investigate whether recurrent abortion was related to vitamin level and the relationship between recurrent abortion and cytokines.Further,according to the vitamin D level grouping,the relationship between vitamin D as an immunomodulator and serum TNF-?,IL-6 and IL-10 cytokines was discussed.Results:1.The relationship between serum TNF-?,IL-6,IL-10,25(OH)VD in the control group and the case group can be seen that the concentration of 25(OH)VD in the case group is lower than that in the control group,P<0.05 Significant.The serum TNF-? concentration in the case group was higher than that in the control group,P<0.01 was extremely significant,and the P value of cytokines IL-6 and IL-10 was >0.05.There was no statistical analysis of serum IL-6 and IL-10 in the case group and the control group.significance.2.According to the vitamin D level group,the variance analysis of IL-6,IL-10 and TNF-?was performed.The results showed that the vitamin D level was related to the serum ytokines TNF-?,IL-6 and IL-10,and vitamin D.The level was increased,and the P value was <0.01,which was statistically significant.Conclusions: The relationship between serum TNF-?,IL-6,IL-10,25(OH)VD in the control group and the case group showed that the 25(OH)VD of the case group was lower than that of the control group,P<0.05 was significant,TNF The-? case group was higher than the control group,and P < 0.01 was significantly different.Recurrent abortion is associated with a lack of vitamin D levels.It is meaningful to supplement vitamin D in pregnant women during pregnancy,and high levels of TNF-? during pregnancy can lead to recurrent miscarriage.Further studies found that vitamin D levels were associated with serum cytokines TNF-?,IL-6,IL-10,TNF-?,IL-6,IL-10 increased with vitamin D levels,and P values were <0.01,the statistical difference is extremely significant.It can be explained that the level of vitamin D as an immunomodulator is related to the pregnancy outcome,but further studies are needed for the supplemental dose of vitamin D during pregnancy and the level of vitamin D combined with serum TNF-? to predict recurrent spontaneous abortion.
