Objective:By using blue laser imaging(BLI)technology and flexible spectral imaging color enhancement(FICE)technology,the coincidence rates of the diagnosis and pathological results of suspicious early esophageal cancer and precancerous lesions under endoscopy were compared and analyzed.To evaluate the diagnostic value of blue laser imaging and expandable electronic spectroscopic color enhancement in early esophageal cancer and precancerous lesions,and to improve the diagnostic rate of early esophageal cancer and precancerous lesions under endoscopy.Methods:From January 2018 to September 2018,the same experienced and highly qualified physician from the Digestive Endoscopy Center of our hospital collected the cases suspected of early esophageal cancer and Barrett's esophagus when he underwent general white-light endoscopy at the Digestive Endoscopy Center of Yuhuangding Hospital,Yantai Affiliated to Qingdao University.According to the AB classification proposed by the Japanese Esophageal Society in 2012 as the diagnostic criterion for evaluating endoscopic images,the patients with early esophageal cancer were diagnosed under endoscopy.The patients with type A were considered as having inflammation and type B as cancerous lesions.Barrett's esophagus is the palisade vessel below the normal esophagus.If the squamous column line moves up,it is the columnar epithelium below.Barrett's esophagus is the palisade vessel visible through the columnar epithelium,which is the diagnostic criterion of Barrett's esophagus under endoscopy.Then,each case was examined by BLI and FICE respectively,and the pathological diagnosis of the lesions was the gold standard.SPSS 17.0 statistical software was used to analyze the data.Technical data were expressed as examples.test was used to evaluate the coincidence rate of FICE and BLI in the diagnosis of early esophageal cancer and Barrett's esophagus.The diagnostic value of FICE and BLI for early esophageal cancer and precancerous lesions was evaluated.Results : This study included 59 cases of suspected early esophageal cancer,12 cases of pathologically confirmed early esophageal cancer,15 cases of early esophageal cancer diagnosed by BLI mode and 13 cases of early esophageal cancer diagnosed by FICE mode.83 cases of suspected Barrett's esophagus were collected.The final pathological diagnosis was Barrett's esophagus in 49 cases,Barrett's esophagus in 52 cases diagnosed by BLI mode and Barrett's esophagus in 48 cases diagnosed by FICE mode.The coincidence rates of BLI andFICE for endoscopic diagnosis and pathology of early esophageal cancer were 94.92%(56/59)and 81.36%(48/59),respectively.There were significant differences between the two groups(?2=4.08,P=0.039,P<0.05).The coincidence rates of endoscopic diagnosis and pathological diagnosis of Barrett's esophagus by BLI and FICE were 96.39%(80/83)and 89.16%(74/83),respectively.There was no significant difference between the two groups(?2=3.13,P=0.07,P>0.05).Conclusion:In the diagnosis of early esophageal cancer,the coincidence rate of endoscopy image made by BLI mode with final pathological diagnosis is higher than that made by FICE mode,and P < 0.05.The difference is statistically significant,which indicates that BLI has better diagnostic effect than FICE on early esophageal cancer.In the diagnosis of Barrett's esophagus,the results showed that the diagnostic coincidence rate of BLI mode for Barrett's esophagus was still higher than FICE mode,but P > 0.05,there was no significant difference.The results suggest that both methods can be selected in clinical diagnosis of Barrett's esophagus,but the diagnostic effect of BLI may be better.Significance:Through this comparative study,clinical doctors can better guide the diagnosis of early esophageal cancer and precancerous lesions,accurately screen high-risk groups,so as to achieve early diagnosis and treatment,can greatly improve the prognosis of patients with early esophageal cancer.
