Objective: To analyze the risk factors of portal vein thrombosis(PVT)in patients with liver cirrhosis,and to provide theoretical basis for clinical prediction of PVT formation.Methods:In this study,214 patients with liver cirrhosis in the affiliated hospital of Qingdao university were collected as subjects,45 patients with PVT were in the thrombus group,and 169 patients without PVT were in the control group.The general and clinical data of the subjects were analyzed.General data included sex and age.Clinical data included related parameters of thromboelastography(R value,K value,alpha angle,MA value,CI value),white blood cell(WBC),neutrophil granulocyte(NEUT),red blood cell(RBC),hemoglobin(HGB),platelet(PLT)?plateletcrit(PCT),mean platelet volume(MPV),activated partial prothrombintime(APTT),prothrombintime(PT),fibrinogen(FIB),international normalized ratio(INR),antithrombin III(AT-III),D-Dimer(DD),serum total protein(STP),albumin(ALB),serum total bilirubin(TBIL),conjugated bilirubin(DBIL),glutamate aminotransferase(ALT),aspartate aminotransferase(AST),gamma-glutamyltranspeptidase(GGT),alkaline phosphatase(ALP),the width of main portal vein(MPV),portal vein velocity(PVV),spleen thickness,the width ofsuperior mesenteric vein(SMV),the width ofsplenic vein(SV)and Child-Pugh classification.Single factor analysis and logistic regression analysis were used to analyze the risk factors of PVT in patients with liver cirrhosis.Results:(1)In univariate analysis,gender,age,WBC,NEUT,RBC,RBC,HGB,PCT,MPV,APTT,PT,INR,AT-III,STP,ALB,GGT,ALP,Child-Pugh were not associated with PVT in patients with liver cirrhosis(P>0.05).MPV width(t=-12.368,P<0.01),PVV(t=7.347,P<0.01),spleen thickness(t=2.478,P <0.05),SMV width(t=5.368,P<0.01),SV width(t=2.860,P<0.01),FIB(t=-2.093,P<0.05),DD(Z=-5.992,P<0.01),PLT(Z=-2.464,p<0.05),AST(t=3.552,P<0.01),ALT(t=2.608,P<0.05),TBIL(t=3.003,P<0.01),DBIL(t=2.750,P<0.01),R value(t=3.314,P<0.01),K value(t=2.744,P<0.01),alpha angle(Z=-3.953,P<0.01),MA value(Z=-1.964,p<0.05),CI value(Z=-3.581,P<0.01)were significantly correlated with PVT in patients with liver cirrhosis(P<0.05).(2)Logistic regression analysis further revealed that elevation of DD and decrease of PVV were independent risk factors of patients with cirrhosis and portal vein thrombosis.The odds ratio(OR)was in 0.632,1.696.Conclusions:The formation of PVT in cirrhosis is related to the widening of MPV,,the decrease of PVV,the increase of spleen thickness,the increase of PLT,FIB and DD,the increase of AST,ALT,TBIL,DBIL,the shortening of R and K values,and the prolongation of alpha angle,MA value and CI value.Serum DD > 3.06 mg/L and PVV < 14.16 cm/s were independent risk factors for PVT formation in cirrhosis.
