Objective To evaluate left atrial(LA)function in essential hypertension patients with different patterns of left ventricular geometric by three-dimensional speckle tracking(3DSTI).Methods Choose a total of 30 cases as a normal control group,and select a total of90 cases of patients with essential hypertension(EH).Essential hypertension patients were divided into four groups according to American society of echocardiography(ASE):normal pattern group(LVN,n=25),concentric remodeling pattern group(CR,n=20),concentric hypertrophy pattern group(CH,n=25),eccentric hypertrophy pattern group(EH,n=20).The left atrial diameter(LADs),left ventricular end diastolic diameter(LVEDd),interventricular septal thickness(IVST),left ventricular posterior wall thickness(LVPWT),left ventricular ejection fraction(LVEF)and the early diastolic(E)and late diastolic(A)transmitral peak velocity were measured by conventional ultrasound,the early diastolic(Em)and late diastolic(Am)tissue velocity of mitral annulus were also measured.And left ventricular mass index(LVMI),relative wall thickness(RWT)were calculated.The left atrial maximal,minimal and pre-systolic volume(LAVmax,LAVmin,and LAVpre),and left atrial global longitudinal strain(GLS),global radial strain(GRS),global circumferential strain(GCS)were measured by 3D-STI.And LA total ejection fraction,active ejection fraction and passive ejection fraction(LAEF,LAaEF,LApEF)were calculated.The parameters among five groups were compared,and correlations between strain parameters and LVEF,E/A,E/Em were determined by Pearson correlation coefficients.Results(1)Systolic pressure,diastolic pressure,LADs ?LVMI?RWT?E/Em in hypertension group were higher than in normal control group,and there are statistically significant difference(P?0.05);EH group LVEF,E/A were lower(P?0.05).(2)LAaEF was higher,and LApEF was lower in LVN group and CR group than in normal control group(P?0.05);CH group LAaEF was higher,LApEF,LAEF were lower(P?0.05);EH group LAaEF,LApEF,LAEF were lower(P?0.05);(3)Compared with the normal control group,the left atrial global peak strains in hypertension group decrease observably(P ? 0.05);The deformation abilities of left atrial wall(GLS ? GCS ? GRS)have correlation with LVEF,E/A,E/Em(P?0.05).Conclusion left atrial booster pump function increased,while reservoir function and conduit function decreased in hypertension patients,and left atrial function change was obviously in the concentric and eccentric hypertrophy pattern group;3D-STI may play an important role in evaluating left atrial functions of hypertension patients by surveying the volume changing,computing the left ejection fraction and tracing the myocardial motion.3D-STI can provide more valuable reference for clinical diagnosis and treatment.
