ObjectiveTo study the correlation of serum 25-hydroxyvitamin D3[25-?OH?D3]level and gene polymorphism of vitamin D receptor?VDR?Fok I in children with Kawasaki disease?KD?.MethodWe collected clinical data of 136 children with KD admitted to the Children's Medical Center,Affiliated Hospital of Qingdao University from Jan.1st,2017 to Dec.31st,2018.According to the presence or absence of coronary artery lesion?CAL?,these children were classified into CAL group?50 children?and non-CAL?NCAL?group?86children?;According to the efficiency of intravenous immunoglobulin?IVIG?,these children were classified into IVIG-sensitive group?108 children?and no-IVIG-response group?28 children?.The control group includes 76 health children.The serum level of25-hydroxy vitamin D was detected by electrochemiluminescence,while the polymorphism of VDR Fok I gene locus was detected by Sanger sequencing.Results1.KD group has significantlylower serum levels of 25-?OH?D3 than the healthy control group?t=18.90,P<0.05?.There was no significant difference in serum 25-?OH?D3 level between CAL group and NCAL group?t=0.13,P>0.05?;There was no significant difference in serum 25-?OH?D3 level between different levels of coronary artery injury?F=1.97,P>0.05?.No-IVIG-response groupgroups had significantly lower serum levels of 25-?OH?D3 than the IVIG-sensitive group?t=2.33,P<0.05?.2.There are three genotypes at the T/C locus of VDR Fok I gene,including wild-type TT,mutant TC and CC.The distribution of the three genotypes has no difference in the KD and the control group?X2=2.08,P>0.05?;The distribution frequency of T and C allele also has no difference in two groups?X2=1.74,P>0.05?.3.TT,CT and CC genotypes of VDR Fok I gene at T/C locus showed no significant difference between CAL group and NCAL goup?X2=1.85,P>0.05?.There was no difference of allele frequency of allele T and C in the two groups?X2=1.78,P>0.05?.4.The distribution frequency of TT genotype of VDR Fok I gene was significantly higher in no-IVIG-response group than IVIG-sensitive group(?X2=3.97,P<0.05?;The distribution frequency of T allele at T/C locusof VDR Fok I gene was significantly higher in no-IVIG-response group than IVIG-sensitive group?X2=5.15,P<0.05?Conclusion1.Vitamin D levels may be related to the pathogenesis of Kawasaki diseaseand no response to treatment increases,but may not related to the possibility of CAL and degree of coronary artery injury.2.VDR Fok I gene polymorphism may not be related to the pathogenesis of Kawasaki disease and the possibility of CAL.TT genotype and T allele may be high risk factors for no-IVIG-response.
