Objectives To explore the clinical value of monitoring intracranial pressure of different sites in severe neurosurgical patients.Methods 1Animal experimental study:30New Zealand white rabbits,only three groups at random,by means of balloon legl system for the rabbit model of parenchymal high cranial pressure water injection of different doses physiological saline in each group,different dose groups of records and the different time points on both sides of the intracranial ptrssure value and change,fo the three groups of animals on both sides of the intracranial pressure data for statistical analysis.2 Clinical study:60 patiants with craniocerebral injury who met the inclusion criteria in the affiliated hospital of north China university of scienceand technology were collected and analyzed,and were divided into group A(ventricle group)and group B(brain parenchyma group)according to the different locations of intracranial pressure monitoring,with 30 patients in each group.Both groups were given the same routine postoperative neurosurgical treatment.The data of mannitol dose and duration of use,the incidence of complications such as electrolyte disturbance and renal function impairment,and the ICP peak and discharge prognosis score of patients in the two groups wee statistically analyzed.Results 1 Animal model experiment: in the rabbit model of cute focal high intracranial pressure,the values of bilateral intracranial pressure were compared.When the intracranial pressure was increased,the values of bilateral intracranial pressure were increased,and the values of intracranial pressure on the affected side were higher than those on the opposite side(P<0.05),which could reflect the real situation of intracranial pressure more accurately.Blateral intracranial pressure increased with the increase of injection dose(P <0.05).The intracranial pressure at different time points was different and showed a trend of further increase with the change of time(P <0.05).2 Clinical case study: general data of the two groups of patients were statistically analyzed,and there was no statistical significance(P > 0.05),so the two groups of patients were comparable.The dose of mannitol was lower in group a than group b(P<0.05).However,there was no significant differene in using time between the two groups,P>0.05.Complicattions such as electrolyte disturbance and renal function damage were compared between the two groups.Group a was less than group b,P>0.05.According to the analysis of ICPH and prognosis of patients in the two groups,the higher the ICP peak value was,the higher the rate of poor prognosis was.There was no significant difference in the ICP peak value and prognosis between group a and group b,P>0.05,indicting that different monitoring sites had no effect on the prognosis of patients.It indicating that different monitoring is helpful to guide the clinical use of mannitol and improve the occurrence of related complications,and the size of ICP peak is related to the prognosis of patients.Conclusions Through experimental research and clinical case analysis,the results of intracranial pressure monitoring in different parts are as follows: after cerebral hemorrhage on one side,both sides of intracranial pressure will increase,both sides of intracranial pressure monitoring is feasible,but the affected side monitoring is feasible,but the affected side monintoring can more accurately reflect the change of intracranial pressure;In clinical practice,intracranial pressure monitoring is helpful for rational use of dehydrating agents,improving the complications caused by dehydrating drugs,facilitating clinicians to evaluate the patient's condition and prognosis,and the effect of ventricular monitoring is better than of brain parenchymal monitoring.Figur:eight;Table:eight;Reference: one hundred and fifteen...
