Serum 25(OH)D3 In Type 2 Diabetes Mellitus With Different Complications And Its Correlation With Insulin Resistance

Objectives To analyze the difference of serum 25(OH)D3 level in patients with type 2diabetes mellitus complicated with different complications.The correlation between25(OH)D3 and insulin resistance was analyzed.It provides a theoretical basis for early assessment and treatment of complications of type 2 diabetes.Methods 246 patients with type 2 diabetes were selected and divided into the T2 DM group(n=54)and the complication group(n=192).Among them,microvascular group: 48cases;Macroangiopathy group: 48 cases;Neuropathy group: 46 cases;Multiple lesions 50 cases.The researchers divided the patients into two groups with or without insulin resistance,among which 185 cases in the non-insulin resistance group were the control group and 61 cases in the insulin resistance group were the case group.After admission,all the researchers underwent examinations and examinations related to diabetic complications to clarify the complications of the patients.General data and laboratory tests were compared among the groups.All data were statistically processed by SPSS17.0.After logarithmic transformation of non-normal continuous variables,continuous variables were expressed as mean standard deviation(sx ±),and discontinuous variables were tested by chi-square test.Mean comparison between groups was performed by independent sample t test or one-way analysis of variance.The risk factors of T2 DM complicated with different complications were analyzed by multinomial disordered Logistic regression,and the risk factors of insulin resistance were analyzed by unconditioned binary Logistic regression.Relative risk was expressed by odds ratio(OR)and 95%CI,P<0.05 was statistically significant.Results 1 Univariate anova showed that there were statistically significant differences in age,course of disease,serum 25(OH)D3,UAER,TG,FC-P and HbA1 c among the five groups(P<0.05).The chi-square test showed that the gender difference between the five groups was not statistically significant(P>0.05),indicating that the gender of each group was comparable.2 With 25(OH)D3<25.96ug/l as the exposure group,25(OH)D3?25.96ug/l as the non-exposure group.The analysis results showed that the occurrence of various complications was related to the serum 25(OH)D3 level,The degree of affinity was from high to low for a variety of lesion groups,microvascular lesion group,neuropathy group and macrovascular lesion group.3 The indicators with statistical significance in the above analysis were taken as independent variables,and the complications were taken as dependent variables to carry out multiple disordered Logistic regression analysis,so as to observe the risk factors of T2 DM patients with different complications.According to the analysis of multinomial disordered Logistic regression,and 25(OH)D3 was still associated with various complications when other confounders were excluded.The degree of affinity was from high to low for a variety of lesions group,neuropathy group,microvascular disease group and macrovascular disease group.4 Independent sample t test showed that age,SBP,TG,FPG,FIN,2hIN,FC-P,HOMA-IR and HOMA-? in the insulin resistance group were significantly higher than those in the non-insulin resistance group(P<0.05).5The level of 25(OH)D3 in the insulin resistance group was lower than that in the noninsulin resistance group,and the difference was statistically significant(P<0.05).The exposure group was 25(OH)D3<25.96ug/l,and the non-exposure group was25(OH)D3?25.96ug/l.The analysis results showed that the proportion of exposure cases in the insulin resistance group was higher than that in the non-insulin resistance group,andthe difference was statistically significant(P<0.05).The risk of insulin resistance in patients with 25(OH)D3<25.96ug/l was 2.616 times higher than that in patients with25(OH)D3?25.96ug/l.6 In presence of insulin resistance as the dependent variable,the above analysis of meaningful indicators as independent variables,unconditioned two classified Logistic regression analysis,the results showed that under the condition of the ruled out other confounding factors,the low concentration of serum 25(OH)D3 is still a risk factor for insulin resistance occurs,with the decrease of serum 25(OH)D3 increased risk of developing insulin resistance.Conclusions 1 The serum 25(OH)D3 level was decreased in the diabetic complication group,and the decrease degree was from high to low in the group with multiple lesions,the group with neuropathy,the group with microangiopathy and the group with macroangiopathy.2 Serum 25(OH)D3 level was correlated with HOMA-IR,and when serum 25(OH)D3 level decreased,HOMA-IR level increased.Figure 0;Table 25;Reference 67...
