Objective : Hepatic encephalopathy(HE)is a neuropsychological disorder syndrome based on metabolic disorders.It can be divided into minimal hepatic encephalopathy(MHE)and overt hepatic encephalopathy(OHE)according to the severity of clinical symptoms.The clinical symptoms of MHE often appear to be normal,and specific neuropsychological tests are needed to detect abnormalities.However,neuropsychological testing has the disadvantages of factors such as race,age and education level,which is difficult to promote in clinical practice.Recently,resting-state fMRI(rs-fMRI)has been widely used to study spontaneous brain function activities.Among them,the amplitude of low frequency fluctuation(ALFF)can effectively detect the local brain function,especially the spontaneous brain function of neurons.The main purpose of this study is to focus on the changes in ALFF values in the relevant brain regions of patients with cirrhotic mild hepatic encephalopathy.Methods: 20 patients with cirrhosis MHE were enrolled in the experimental group and 20 healthy controls in the control group.The gender,age,years of education,digital connectivity test-A(NCT-A)and digital symbol test(DST)of the MHE group and the healthy control group were statistically tested using SPSS 19.0 software.The rest-state fMRI scan was performed using GE 3.0T magnetic resonance scanner.The image obtained by scanning was post-processed based on the rest-plus software running in Matlab environment.The difference of ALFF values between the two groups was statistically analyzed by two-sample t-test.Correlation between psychometric hepatic encephalopathy score(PHES)and ALFF values in the MHE group was evaluated using Pearson correlation analysis.Results: In the chi-square test,there was no significant difference in gender factors between the MHE group and the healthy control group(P>0.05).In the two-sample t-test,there was no significant difference in age and education between the MHE group and the healthy control group(P > 0.05).The difference between NCT-A and DST was statistically significant(P<0.05).Research in functional magnetic resonance imaging of resting brain shows,compared with the healthy control group,the ALFF value of the right iliac crest,the left anterior gyrus,the right lateral triangle,the left iliac crest,the left insula,and the left inferior temporal gyrus reduced in the MHE group(P<0.05).And the ALFF value of the left margin,the right wedge,the right cerebellum,the right central posterior,the right temporal gyrus,the right central anterior gyrus,the right margin upper back,the right upper anterior gyrus increased(P< 0.05).According to Pearson correlation analysis,there was no significant correlation between the PHES scores of the MHE group and the ALFF values of the brain regions with significant differences(P>0.05).Conclusions: The two PHES scales used in this paper suggest that there are differences in NCT-A and DST results between the cirrhotic MHE group and the healthy control group,which also confirms that MHE may potentially change human cognitive function.In this paper,the ALFF technique of resting-state functional magnetic resonance imaging find that the brain function of patients with cirrhosis MHE had abnormal changes that could not be found in the conventional MRI sequence.Changes in brain activity in these areas may be the basis of neurological function in patients with cirrhotic MHE who have clinical manifestations(following defects,decreased visual spatial processing capacity,decreased exercise capacity,and impaired memory).
