Objective: To explore the relationship between serum CD146 and SV by comparing the serum CD146 levels among three groups: systemic vasculitis(SV),essential hypertension(EH)and health control(HC),in order to provide a new diagnostic marker for the diagnosis and evaluation of SV.Methods: 53 patients with SV,including ANCA-associated vasculitis(AAV),polyarteritis nodosa(PAN)and Takayasu arteritis(TA),who were definitely diagnosed in the Hypertension Center of Xinjiang Uygur Autonomous Region People's Hospital from January 2013 to December 2017,were enrolled.Essential hypertension(EH)and healthy controls(HC)matched by sex and age was collected during the same period.There were 46 patients with EH and 53 HC in this study.Enzyme-linked immunosorbent assay(ELISA)was used to determine the level of serum CD146.Results: There were significant differences in serum CD146 among SV,EH and HC groups.Serum CD146 levels were significantly increased in patients with SV compared with EH and HC [SV vs EH(68.67±45.37)vs(36.83±17.35)ng/m L,P<0.001;SV vs HC(68.67±45.37)vs(37.89±13.99)ng/m L,P<0.01],and there was no difference could be found between EH and HC [(6.83±17.35)vs(37.89±13.99)ng/m L,P=0.983] In the same period,serum CD146 levels were higher in SV patients with renal injury in the sharp contrast to non-renal injury patients [(8.76±49.97)versus(53.78±33.37)ng/m L,P=0.019].Among the subsets of SV,serum CD146 levels were significantly elevated in all SV subsets compared with HC[AAV vs HC(69.65 ± 55.31)vs(37.89 ± 13.99)ng/m L,P=0.015;PAN vs HC(66.44 ± 38.81)vs(37.89±13.99ng/m L),P=0.001;TA vs HC(73.85±41.64)vs(37.89±13.99)ng/m L,P=0.002].Furthermore,serum CD146 levels were significantly higher in AAV patients with renal injury compared to non-renal injury patients [(96.20 ± 64.33)vs(43.09±26.72)ng/m L,P=0.007].Whereas,serum CD146 levels were slightly but not significantly higher in PAN and TA patients with renal injury than in those with non-renal injury(all P>0.05).In addition,there were significantly positive correlation between serum CD146 levels and white blood cells(WBC),serum creatinine(Scr),blood urea nitrogen(BUN)in SV patients[r=0.462,P<0.001;r=0.287,P=0.037;r=0.271,P=0.049].Moreover,Serum CD146 levels in AAV patients were also positively correlated with Scr and BUN [r = 0.729,P < 0.001;r = 0.602,P = 0.005].The best cutoff value of serum CD146 levels was estimated by calculating ROC curves.For the diagnosis of SV by analysis the serum CD146 levels between SV and HC,the best cutoff value was at 45.05 ng/m L.The sensitivity and specificity were calculated as 64.2% and 77.4%.Furthermore,the area under curve(AUC),positive predictive value,negative predictive value,positive likelihood ratio,negative likelihood ratio was 0.740,98.2%,10.2%,2.83 and 0.46,respectively.Conclusion: Serum CD146 levels in SV group were higher than those in EH group and HC group,especially in AAV with renal injury group.CD146 may be a potential marker of AAV with renal injury.
