Objective: The DKK1(Dickkopf1)gene polymorphism in patients with type 2 diabetes mellitus(T2DM)in Shihezi area was determined and analyzed for its relationship with bone metabolism,which provided a theoretical basis for the clarification and prevention of the disease mechanism.Methods: 1.136 outpatients and inpatients from the hospital in Shihezi area from October 2016 to October 2017 were selected.According to the medical history,glucose tolerance test and bone mineral density test results,they were divided into four groups: normal glucose tolerance and normal bone mass(A Group of 26 patients),normal glucose tolerance with abnormal bone mass(28 patients in group B),type 2 diabetes with normal bone mass(27 patients in group C),type 2 diabetes with abnormal bone mass(55 patients in group D).2.Determine and record general clinical indicators such as age,height and weight;determine the glycosylated hemoglobin(Hemoglobin A lc,HbA1c)by high pressure liquid phase method,and measure blood calcium(Ga),blood phosphorus(P)and glycerol by automatic biochemical analyzer.Biochemical indicators such as triglyceride(TG)and fasting plasma glucose(FPG);dual energy X-ray(DEXA)for the determination of BMD(femoral neck and L1-4).3.The distribution of polymorphisms of DKK1 gene rs1373004,rs1896367,rs1528877 and rs2241529 loci were detected by time-of-flight mass spectrometry.The genotypes and allele frequencies were compared between groups.4.Data processing was performed using SPSS17.0 statistical software.The variables were expressed in(sx ±)form.The chi-square test was used to determine whether the genotype distribution of the single nucleotide polymorphism(SNP)locus of DKK1 gene accords with the Hardy-Weinberg equilibrium law.If the baseline data were homogeneous,the t-test was used for the measurement data between the two groups,and the variance analysis was used for comparison between groups;if the baseline data were not uniform,between the groups Data were compared using covariance analysis(P < 0.05,the difference was statistically significant).Results: 1.Compared with group A,the age and menopausal age of group B and group D were statistically different(p<0.05),indicating that the baseline data were not uniform,and the covariance analysis was used to compare the clinical indicators between groups.2.After covariance analysis,compared with group A(5.31±2.18,5.95±0.87),FPG(7.69±3.20,7.42±1.86)and HbA1c(7.21±1.34,7.48±1.14)levels in group C and group D were increased.(P<0.05);Compared with group A(1.19±0.13,0.90±0.12),BMD levels of group B and group D in L1-4(0.84±0.17,0.91±0.12)and femoral neck(0.71±0.10,0.74±0.11)were decreased;Compared with group A(2.51±1.76),the lipid metabolism index TG(1.51±0.98,1.70±1.00)in group B and group D were decreased(P<0.05).3.The genotype distributions of DKK1 gene rs1373004?rs1896367? rs1528877 and rs2241529 were in accordance with Hardy-Weinberg's law of genetic balance(P>0.05).4.Comparison of genotype and allele frequency distribution of DKK1 points: Compared with group A,the distribution of rs1373004 loci in group B and group D was statistically different;compared with group A,The distribution of rs1896367 loci in group B,group C and group D was statistically different;Compared with group A,the distribution of rs1528877 and rs2241529 loci in group C and group D were statistically different(P<0.05).5.Comparison of general clinical data and clinical metabolic indexes between different genotypes of DKK1 gene: DKK1 gene rs1373004 locus in group D CC type(3.390±1.048)LDL-C is lower than CA+AA type(4.835±0.035),P<0.05;The rs1896367 locus in group A,the menopausal period of GG type(17.632±7.712)was higher than AG+AA type(10.571±6.451).The level of P and LDL-C from group A in GG type(1.059±0.086,2.747±0.908)were lower than in AG+AA type(1.143 ± 0.051,4.067 ± 0.769),P<0.05;In the D group,the GG type(4.835±0.035)LDL-C was higher than the AG+AA type(3.389±1.038),P<0.05;The rs1528877 locus in group D,the level of ALP in AA type(54.000±15.800)was lower than AG+GG(75.900±21.819),P<0.05.The rs2241529 locus in group D,AA type(4.835±0.035)LDL-C was higher than in AG+GG type(3.445±1.045),P<0.05.Conclusion: 1.In T2 DM postmenopausal women in Shihezi,Xinjiang: the frequency distribution of DKK1 gene rs1528877,2241529 genotypes and alleles may be related to glucose metabolism,rs1373004 locus,rs1896367 locus genotype and allele frequency distribution may be related to glucose metabolism and Bone metabolism.2.Gene mutations in the DKK1 gene rs1373004,rs1896367,and rs1528877 may be associated with disorders of lipid metabolism in postmenopausal women with T2 DM.
