ObjectiveTo assess the application value of Diffusion-weighted imaging?DWI?and apparent diffusion coefficient?ADC?in the diagnosis of diabetic foot osteomyelitis.its correlation with clinical parameter ABI?TBI and TcPO2.MethodsThis study was carried out on thirty patients with the clinical comprehensive diagnosis of diabetic footosteomyelitis,24 males,6females with mean a age of 61.76±17.11 years were enrolled in this study,the average diabetes foot disease duration was9.27±5.95 years,the mean HbA1c level was 10.32±4.18%.All patients underwent conventional MRI and DWI examinations.ADC value of the infected feet were evaluated.The results of two groups were compared by student-test for statistic analysis.The receiver operating characteristic curve was drawn to evaluate the value of ADC for the diagnosis of diabetic osteomyelitis.ResultsThe signal of normal bone on DWI was hypointense,while the signal of osteomyelitis was hyperintense.The mean ADC of normal bone was0.88±0.13×10-3mm2/s,and the mean ADC of osteomyelitis was1.48±0.17×10-33 mm2/s,the difference of the two group was statistically significant.The area under the ROC curve was 1.23×10-33 mm2/s.As the ADC value of as a threshold,the sensitivity and specificity were 96.7%?96.7%and 99.2%respectively in diagnosis of osteomyelitis.ConclusionDWI compared to conventional MRI with short imaging time and no requirement of contrast agent.Therefore,DWI may be an alternative diagnostic method for detection of osteomyelitis and measuring the ADC values can quantitative evaluate diabetic foot osteomyelitis.
