Investigation On Current Status Of Medication Therapy In Patients With Hypertension Complicated With Atrial Fibrillation

Objective: To investigate the current status of medication therapy in patients with hypertension complicated with atrial fibrillation,in order to get better guidance of the clinical therapy.Methods: A total of 590 hypertension combined with atrial fibrillation patients hospitalized in the first affiliated hospital of Chongqing Medical University from January 1,2014 to December 31,2016.Recorded basic information,drug treatment and international normalized ratio.The patients were divided into three groups according to age: group A(< 65 years old),group B(65-74 years old),and group C(? 75 years old).According to the statistical analysis of the data of patient's medication regimens during hospitalization,the current situation of clinical treatment was reflected.Results: ACEI/ARB was used in 547 patients with hypertension complicated with atrial fibrillation(92.71%).?-RI in 480 cases(81.36%);calcium channel blockers in 250 cases(42.37%);diuretics in 290 cases(49.15%),including 178 cases(30.17%)of spironolactone;statins in 334cases(56.61%).ACEI/ARB+?-RI combination accounted for 167 cases(28.31%).The blood pressure of 380 cases(64.41%)reached the standard,among which the ACEI/ARB+?-RI +CCB+ diuretic group had the highest blood pressure compliance rate of 94.67%.The heart rate of 525 patients(88.98%)reached the standard,and the heart rate of the drug regimen ?-RI+CCB group was the highest 100%.A total of 543 cases(92.03%)were hospitalized with CHA2DS2-VASc?2 scores,among which 255 cases(46.96%)were treated with anticoagulant therapy.There were 197 cases(36.28%)of warfarin,and the compliance rate of INR was 17.76%.Inpatients were treated with new oral anticoagulant in 84 cases(14.24%),from 2014 to 2016,the usage rate was 0,8.38% and 21.40% respectively.Conclusion: ACEI/ARB is the most commonly used antihypertensive therapy in patients with hypertension complicated with atrial fibrillation.Combined antihypertensive regimen can make patients' blood pressure and heart rate reach the standard better.Most patients need anticoagulant therapy,but the rate of anticoagulant therapy is low.Use warfarin,the rate of INR meeting the standard is at a low level.The application of novel oral anticoagulants is increasing.
