Research On The Construction Of Medicalalliance In A Hospital Of Chongqing

Objectives: To research the status quo of medical alliance in the first affiliated hospital of Chongqing medical university and summarize the experience and deficiencies,using the method of empirical research.The article combine the experience of the first affiliated hospital with the problems in the implementation process of Chongqing medical alliance,so as to put forward countermeasures and suggestions to provide reference for the promotion of primary diagnosis and two-way referral,and promote the formation of hierarchical diagnosis and treatment pattern in Chongqing.Methods:(1)Literature analysis: The Chinese and foreign databases,such as CNKI,Wanfang,and Vip,were used to consult the literature related to this study.It was necessary to master the research achievements of domestic and foreign scholars on medical alliance and lay a theoretical foundation for the research.(2)Case analysis: The first affiliated hospital was selected to be a typical case.This paper summarized the experience and shortcomings of medical alliance,to put forward corresponding countermeasures and suggestions for the construction of medical alliance inChongqing.(3)Summary and induction: An in-depth study on the construction of medical alliance in the first affiliated hospital was conducted.The article proposed relevant countermeasures based on the highlights of the first affiliated hospital and the problems existing in Chongqing medical alliance.Results: In terms of urban medical groups,the first affiliated hospital built a tertiary medical group with “1+3+15+5” pattern.In terms of medical community,the custody hospital formed a medical community with one hundred and seventy-nine township hospitals and community health service centers.In terms of telemedicine collaboration network,one hundred and sixty medical institutions joined the medical cooperation network,covering thirty-six districts and counties of Chongqing and Sichuan,Guizhou and Hainan provinces,with a total service volume of more than two hundred and fifty-one thousand cases.In terms of specialized alliances,there were sixteen specialized alliances,which covered ten provinces and cities including Chongqing,Sichuan and Yunnan and others.In terms of two-way referral,two thousand four hundred and forty-four patients were transferred to the first affiliated hospital,and two thousand four hundred and fifty-one patients were transferred to primary health care institutions.In terms of medical expense,the average expense of outpatient and emergency for per visit were five hundred and thirty-three point four one yuan,and the average hospitalization expense were nineteen thousand seven hundred andeighty-three point six one yuan.In terms of teaching and scientific research,the custody hospital established one hundred and ninety-two key and characteristic specialties and developed one thousand five hundred and forty-seven new and characteristic technologies.There were seven hundred and thirty-two scientific research projects and two thousand nine hundred and seventy-two papers.Seventeen thousand six hundred and fifty-seven apprentice and trainees were trained by the first affiliated hospital.Conclusion: The construction of medical alliance of the first affiliated hospital made a great effect.The system standards,medical service,digital information,expert resource,first aid network,hospital development,culture construction and financial management of the member in medical alliance realized integration.The management level,diagnosis and treatment level,integrated service capabilities,scientific research ability,discipline construction and talent team construction of custody hospitals within the medical alliance would be significantly improved.The experience of the first affiliated hospital on medical alliance construction provided rich experience for Chongqing.However,two-way referral lacked unified referral standards and there were no clear referral process,supervision system and incentive mechanism.Moreover,the doctors had low enthusiasm to referral patients,and the hospital departments did not pay attention to it.The effect on the vertical integration of resource,reasonable diversion of patient and medical cost control needto be improved.Under the premise of clarifying their own responsibilities,rights and interests,hospitals at all levels should cooperate with each other,so as to improve the service ability of primary medical institutions and implement primary diagnosis and two-way referral effectively to further promote the realization of hierarchical diagnosis and treatment.
