Curative Effect Analysis Of Mitral Valvuloplasty In The Treatment Of Children With Mitral Insufficiency

Objective: To investigate the clinical effect of mitral valvuloplasty in the treatment of children with mitral insufficiency.Methods:135 cases patients accepted mitral valvuloplasty form2008.7.1 to 2018.7.1 in Children ' s Hospital of Chongqing Medical University were analyzed by reviewing of the clinical materials,including55 males and 80 females.The surgical techniques used for mitral valve repair included commissural closure(90),shrink the annular dilatation(13),cleft closure(29),edge-to-edge technique(20),chordal shortening technique(5),triangular resection(5).The concomitant cardiac anomalies were treated simultaneously.The results of surgery were assessed by comparing the severity of mitral regurgitation,left atrial diameter(LAD),left ventricular and diastolic diameter(LVDd),left ventricular and systolic diameter(LVDs)and ejection fraction(EF)of pre-operation and post-operation through the paired t test.Results: All children completed the operation successfully,and no deaths occurred during the operation.The in-hospital mortality rate was5.2 %,and secondary operation rate was 3.1%.The remaining 124 children were followed up in clinic,missing rate was 8.9%.The average follow-up was(18.2±15.2)months.The average LAD(20.0±5.8)mm,LVDd(33.7± 6.3)mm,LVDs(21.6 ± 4.9)mm of post operative children reduced remarkably(P=0.000),and the EF(68.7 ± 4.6)% ishigher than those before operation(P=0.000).The difference was statistically significant.The rate of less regurgitation after mitral valve repair was 56.6%.Conclusions:The mitral valvuloplasty have good operative effects and fewer complication in the treatment of children with mitral insufficiency,and it can be used as the first choice.If there are other intracardiac anomalies,good effect could be obtained after operating at the same time.
