PurposeCompare the clinical characteristics,pregnancy outcome and laboratory index between preeclampsia pregnant women and normal women.Explore the expression level of inflammatory factors in serum of pregnant women with early onset of severe preeclampsia.Method1 From January 2016 to January 2018,160 single pregnancy preeclampsia patients of Han nationality and 160 normal pregnant women as the control group were selected from the Department of Gynecology and Obstetrics,affiliated of Southeast University.The preeclampsia prediction model was established.2 From July 2017 to December 2017,15 patients with early onset of severe preeclampsia and15 normal women were selected.All the subjects were analyzed by clinical epidemiology and clinical outcomes.Detection of NLRP3,Caspase-1,IL-1?and IL18 in serum of two groups by ELISA.3 The SPSS 20.0 statistical software was used to analyze the data.Outcomes1 Clinical characteristics:there were significant differences in age,partner age,education,weight,BMI,weight before pregnancy,BMI before pregnancy,SBP,DBP and family history of hypertension?P<0.05?.2 Laboratory index and pregnancy outcome:there were significant differences in ALT,LDH,ALB,Cr,UA,MPV,TT,APTT?P<0.05?.there were differences in delivery weeks,cesarean section rate,neonatal weight and Apgar scores,birth rate of premature infants,postpartum hemorrhage rate,incidence of low birth weight infants,neonatal asphyxia rate,abruptio placentae and HELLP syndrome.?P<0.05?.3 Logistic regression analysis showed that BMI,family history of hypertension,uric acid,LDH,MPV were independent predictors of preeclampsia.The establishment of Logistic regression model,Logit(PPE)=-15.559+0.238?BMI before?+1.244?hypertension family history?+0.012?LDH?+0.010?UA?+0.375?MPV?.4 Through the ROC curve,It was found that UA had a high diagnostic value.The area under the curve was 0.762.The Cut Off value was 341.5 umol/L.The diagnostic sensitivity and specificity were 62.6%and 79.6%,OR was 6.5.5 The expression of NLRP3?Caspase-1?IL-1??IL-18 in serum of pregnant women with early onset severe preeclampsia was higher than that of normal pregnancy,and the level of IL-1?was about 2 times higher?P<0.05?.Conclusion1 Family history of hypertension,high BMI before pregnancy,high LDH,MPV,UA were independent risk factors of preeclampsia,and UA had the highest diagnostic value for preeclampsia.2 There were significant differences in ALT,LDH,ALB,Cr,UA,MPV,TT,APTT?P<0.05?.The rate of iatrogenic premature delivery,cesarean section,low birth weight and neonatal asphyxia were significantly higher in preeclampsia?P<0.05?.3 The expression of IL-1?in maternal serum of preeclampsia group was significantly higher?P<0.05?.NLRP3?Caspase-1 and IL-18 in preeclampsia were higher?P>0.05?.The pathogenesis of early-onset severe pre-eclampsia should be further explored.
