Objective:To study the characteristics of iron metabolism in patients with chronic kidney disease stage 3-5 and different genotypes of thalassemia.Methods:(1)Atients with chronic kidney disease in the department of nephrology of Affiliated Hospital of YouJiang Medical University for Nationalities,who met the diagnostic criteria for chronic kidney disease stage 3-5 in line with K/DOQI guidelines were selected for complete set of hemoglobin electrophoresis and genetic diagnosis of thalassemia,according to results,the patients were divided into chronic kidney disease stage 3-5 group: 30cases(group B),chronic kidney disease stage 3-5 with ?-type thalassemia group: 30 cases(group C),chronic kidney disease stage 3-5 with ?-type thalassemia group: 30 cases(group D)according to the presence or absence of thalassemia and different genotypes.At the same time,30 healthy people were selected as healthy control(group A).(2)Clinical data and blood samples were collected from all the selected patients,blood samples were tested for Serum iron(SF),Serum iron(SI),soluble transferrin receptor(sTfR),and total iron binding force(TIBC),Calculate the Transferrin Saturation(TSAT)using the formula,TSAT=(SI/TIBC)*100%.(3)All clinical data and test results were statistically analyzed.Results:(1)In the SI test results,group B,C and D were all lower than group A,P <0.001,group C and D were lower than group B,P <0.05,the difference was statistically significant.There was no significant difference between group C and D,P > 0.05.(2)In the SF test results,group B,C and D were significantly higher than group A,P <0.001,group C and D were higher than group B,P<0.05,the difference was statistically significant.Therewas no significant difference between group C and D,P > 0.05.(3)In sTfR test results,group A,C and D were lower than the healthy control group B,P < 0.05,the difference was statistically significant.However,there was no significant difference among the group A,C and D,P > 0.05.(4)In the TIBC test results,group B and C were lower than group A,group D was higher than group B P <0.05,the difference was statistically significant,there was no significant difference among the group A and D?group B and C?group C and D,P > 0.05.(5)TSAT results were calculated and compared,group B was lower than group A,P=0.002,the difference was statistically significant.while there was no significant difference among the group B,C and D,P > 0.05,the difference was no statistically significant.Conclusion:(1)Patients with chronic kidney disease stage 3-5,chronic kidney disease stage 3-5 with thalassemia,have varying degrees of iron deficiency.Patients with chronic kidney disease stage 3-5 with thalassemia have less iron deficiency than those with chronic kidney disease stage 3-5.(2)There was no significant difference in iron deficiency in patients with chronic kidney disease stage 3-5 with different genotypes of thalassemia.(3)Hyperferricemia is prevalent in patients with chronic kidney disease stage 3-5(including chronic kidney disease stage 3-5 with thalassemia).
