Effect Of Gastric Bypass On The Relationship Between Serum Orexin Expression And Insulin Resistance In Type 2 Diabetic Rats

Objective At first,the task of model making in type 2 diabetic rats was improved.Then,the expression of Orexin and its relationship with insulin resistance were observed after gastric bypass in type 2 diabetic rats,and the mechanism of gastric bypass in the treatment of type 2 diabetes was further clarified.Methods Sixty standard rats were selected and randomly divided into two groups,of which 40 were diabetic group(DM group)and 20 were as control(NC group).The diet of DM group was lard,sucrose,egg yolk,basic feed,and DM group was fed with high fat and high sugar for 4 weeks.1%(STZ)solution of streptozotocin was injected intraperitoneally with low dose(35 mg/kg)to construct model.Based on the successful rats,15 rats were randomly assigned to the operation group(RYGB group).There were 2 cases of postoperative infection and 15 cases of sham operation group(DM group).Gastric bypass surgery was performed in the operation group and in situ surgery was performed in the sham operation group.The changes of whole blood glucose and serum insulin were measured by blood glucose analyzer and immunoassay.Orexin and TNF-? in serum need enzyme linked immunosorbent assay(Elisa).At the end of 4 weeks,the brain tissues were collected for immunohistochemistry and quantitative detection of Orexin expression.Results(1)In gastric bypass group,fasting blood glucose decreased significantly before operation and 4 weeks after operation,and the decrease rate was 30%.Compared with before operation,the blood glucose of rats in sham operation group was slightly higher than that before operation(p < 0.05),but the control group did not change much at the fourth week after operation.(2)In the gastric bypass group,the degree of insulin resistance showed a tendency to drop at the 1st week after operation,and the fall amplitude was about 20%.Significantly lower than before(p < 0.05),the remaining two groups were indifferent.(3)The plasma levels of Orexin in RYGB group,DM group and NC group were measured before operation,and the Orexin level in NC group was significantly higher than that in the other two groups and TNF-? decreased significantly compared with the other two groups.The Orexin level in RYGB group and DM group was significantly increased after different surgical intervention.The level of Orexin in the RYGB group was significantly higher than that in the preoperative Orexin group 4 weeks after operation(p < 0.05),but there was no significant difference between the other two groups compared with the preoperative Orexin level(p > 0.05).At the same time,the expression of Orexin was better in the gastric bypass group than in the DM group(p < 0.05),and the expression of Orexin in the DM group was slightly better than that in the NC group(p < 0.05).Conclusions(1)Gastric bypass was effective in the treatment of type 2 diabetic rats.(2)Gastric bypass combined with Orexin was involved in the treatment of type 2 diabetes.(3)Gastric bypass has a positive effect on the expression of Orexin,while Orexin can reduce the inflammatory effect of TNF-? and thus inhibit insulin resistance.
