Objective: This study investigated the clinical pathological features of patients with primary liver cance with diabetes,and screened differential methylation regions between primary liver cancer patients with diabetes and primary liver cancer patients with no-diabetes by whole-genome methylation sequencing technology,in order to clarify the role of diabetes as a risk factor in the development of liver cancer,and provide evidence in primary liver cancer for early diagnosis,prevention and treatment.Methods: The clinical information of patients with liver disease in Quanzhou 910 Hospital was collected from November 2016 to August 2018.The clinical data of 40 patients with primary liver cancer with diabetes and 376 patients with primary liver cancer with no-diabetes were collected.It was retrospectively analyzed.Blood samples from 7 patients of primary liver cancer with diabetes and 15 patients with primary liver cancer with no-diabetes were collected.Whole genome DNA was extracted,and DMR was obtained by whole-genome methylation sequencing analysis for GO and KEGG enrichment analysis.Finally,the enrichment results were analyzed and verified in a public database.Results: 1.The clinical data analysis of primary liver cancer with diabetes and primary liver cancer with no-diabetes showed that the levels of age,BMI and ALT statistically different(P<0.05),and the tumor marker CA199 was statistically different(P< 0.05),and the primary liver cancer with diabetes group was higher than the primary liver cancer with no-diabetes group.2.The data of whole-genome methylation sequencing revealed that the methylation site was mainly concentrated on the CG sequence.In the gene function regions,the methylation of the CG sequence was at a high level,and the methylation level of the CGI region was the lowest;in the downstream 2Kbp regions,the methylation level of the gene body region was the highest.the distribution analysis of DMR in the gene function regions revealed that DMR was mainly concentrated in the intron region.The analysis of DMR-related gene GO and KEGG enrichment found that it was mainly involved in biological processes such as cells and nerves,morphine addiction pathways.3.The differential genes of ADCY1 and GNAO1 were screened according to the enrichment results,which were confirmed to be significantly low in liver cancer tissues and high methylation levels in the promoter region of liver cancer tissues.Conclusion: 1.Diabetes is an age-related disease,and the combined detection of related indicators can improve the detection accuracy of early primary liver cancer.2.This study constructed a genome-wide DNA methylation map with single-base resolution of primary liver cancer with diabetes and primary liver cancer with no-diabetes.The morphine addiction pathway as a breakthrough,the mechanism of primary liver cancer and diabetes was investigated and early diagnosis and prevention of primary liver cancer provided ideas.3.Genes of ADCY1 and GNAO1 provided the possibility to study the relationship between primary liver cancer and diabetes,and provide new targets for the discovery of biomarkers related to the development of liver cancer and the treatment of liver cancer.
