Objective:The method of microsurgery and endovascular interventional surgery was used to treat postcirculation aneurysms,and the clinical efficacy of the two treatment options was compared to discuss the clinical treatment of postcirculation aneurysms.Methods:We selected 1720 cases of intracranial Aneurysms with intracranial Aneurysms from December 1997 to December 2016,of which 38 patients were from the Posterior circulation artery Aneurysms,and the total number of patients with aneurysm rupture was 35 cases of Subarachnoid hemorrhage(Subarachnoid),and 3 cases were not ruptured,and all of them were treated with craniotomy Aneurysms,and good clinical efficacy was obtained.In the case of the 35 patients,the clinical manifestations,imaging,surgical procedures,surgical procedures,and the complications,the treatment and analysis.We select 45 cases to be treated with endovascular interventional surgery the aneurysm from December 2006 to December 2016 which is to be analyzed for the patient's pre-op status,the hospital time,the treatment time,the timing of the procedure,and the procedure of the procedure.Results:Thirty-eight patients were treated with microsurgery to find 41 aneurysms,a successful clip of 40,a single inoperable clot in the cavity of a tumor,and when the procedure was separated,10 of them broke,and 31 of them didn't rupture.Postoperative follow-up and GOS rating,38 cases in 33 cases were cured,? self-care level 3 cases,2 cases died of brain edemaForty-five aneurysm with intravascular interventional treatment,38 patients were successfully treat with disposable embolism,and that angiography of the aneurysm cavity after embolization showed satisfactory filling of the aneurysm cavity with satisfactory operation effect.Of the 45 patients treated with intravascular therapy,38 were cured,4 had postoperative limb movement disorder,symptomatic relief after symptomatic treatment,2died,and 1 suffered severe disability.Conclusion:Intracranial posterior circulation aneurysms,due to deep location,difficult exposure,limited surgical space,high rates of big and large aneurysms,proximity to the brain stem and cranial nerves,etc,are still the problems for the neurosurgeons.The treatment principles for the PCA should be individualized.With the reasons of deep location,difficulty in exposure,old age,poor health,high hunt-hess grade,and poor surgical tolerance,Intravascular intervention should be considerated.Microsurgical treatment has obvious advantages in the cases of superficial location,easy exposure,irregular shape or need for vascular bypass,and hematoma in the brain has mass effect and needs to be removed.
