Objective:To evaluate the effects of fascia iliaca compartment block combined with dexmedetomidine before anesthesia of femoral neck fracture in elderly patients with hypertension.Methods:A total of 40 elderly hypertensive patients(the gender was not limited,age 65~85years old,40~72 kg,ASA grade ?~?)with femoral neck fracture were selected and randomly divided into research group and control group(n=20,each).The research group underwent fascia iliaca compartment block(FICB)combined with intravenous infusion of dexmedetomidine before spinal anesthesia,and the control group only underwent fascia iliaca compartment block.A sensory test was performed every 10 minutes after FICB completed.If the VAS score was ?4 before positioning,added fentanyl 0.5?g/kg intravenously to keep the VAS score <4.MAP and HR were recorded at entering waiting room(T0),leaving waiting room(T1),after moving to operating bed(T2),after lateral position(T3),and after successful puncture(T4).The blocking efficiencies of 10 min and20 min were recorded.The number of people using fentanyl was recovered and the anesthesia satisfaction of patients was recorded.Results:Compared with T0,the MAP and HR of control group increased in T2-T4(P<0.05),the MAP and HR of research group decreased in T1-T4(P<0.05);Comparison between groups: There was no significant difference in T0 between the two groups(P>0.05),the MAP and HR of research group was lower than that of control group in T1-T4(P<0.05).The blocking efficiencies of the research group was higher than that of the control group at 10 min(P<0.05).The research group required fewer fentanyl remedies and fewer cases of hypertension(P<0.05);anesthesia satisfaction of patients was higher than that of the control group(P<0.05).Conclusion:The use of fascia iliaca compartment block combined with dexmedetomidine in elderly hypertensive patients with femoral neck fracture before spinal anesthesia can shorten the onset time,enhance the analgesic effect,make the hemodynamics more stable,and improve the patient's anesthesia satisfaction.
