Objective:The shear wave elastography(SWE)color mode of thyroid nodules was analyzed,and the diagnostic value of SWE color scoring system for thyroid nodules was explore.Methods:A total of 261 patients who underwent thyroid nodule surgery and confirmed by operation and pathology in our hospital from January 2017 to February 2018,underwent routine and SWE ultrasonography.Analyzing the SWE color pattern of all thyroid nodules,selecting the indicators that can evaluate benign and malignant nodules according to the frequency of occurrence of each color pattern in the total sample,and screening out statistically significant indicators;giving different indicators to each indicator the scores were established,and the SWE color scoring system was established.The ROC curve was used to evaluate the diagnostic value of SWE color scoring system,two-dimensional ultrasound and the combination of the two for thyroid nodules.The nodules were divided into <1cm?1cm-2cm?2-3cm according to the diameter of the nodule.Three groups were used to evaluate the diagnostic value of SWE color scoring system by ROC curve,and to analyze the influence of nodule size on SWE color scoring system.Results:1.The difference five SWE color modes(the hardest color of the nodule,the maincolor inside the nodule,whether the nodule has a hard ring,the internal color consistency of the nodule,the color consistency of the nodule and the surrounding gland)was statistically significant in the benign and malignant nodules of the thyroid gland(P<0.05).The hardest part of the benign nodules is more common in blue,the main internal colors are mostly blue,there is no hard ring,the internal colors are more consistent,and the color of the surrounding glands is more consistent,while the hardest part of the malignant nodules is orange.The main internal colors are mostly green,many hard rings,inconsistent internal colors,and inconsistent with the surrounding glands.2.Using 0-5points as the diagnostic criteria,calculate the sensitivity and specificity of different SWE color score boundary points in the diagnosis of thyroid malignant nodules,and use 3points as the standard for diagnosing thyroid malignant nodules.3 points is the optimal cut-off point.3.The SWE color scoring system diagnoses malignant thyroid nodules,the sensitivity,specificity,and accuracy of the thyroid malignant nodules are higher than those of the SWE color mode,The area under the ROC curve is large(79.8%,81.2%,80.1%,0.846);The SWE color scoring system and two-dimensional ultrasound diagnosis of thyroid malignant nodules were more sensitive,specific,and accurate,and the area under the ROC curve was larger(88.7%?71.8%?86.2%?0.912).4.According to the size of thyroid nodules,calculate the sensitivity and specificity of different SWE color score boundary points in the diagnosis of thyroid malignant nodules.The optimal cut-off point of nodule diameter <1cm,1-2cm,2-3cm is They are 2 point,3 point,and 4 point respectively.When the nodules are not grouped,the best cutoff point is 3 points.Conclusion:1.SWE color pattern of benign and malignant nodules has certain characteristics,which has certain value for identifying benign and malignant nodules.2.The SWE color scoring system has higher accuracy in diagnosing thyroid malignant nodules than SWE color modes.3.SWE color scoring system combined with two-dimensional ultrasound is more accurate than SWE color scoring system and two-dimensional ultrasound in thyroid malignant nodules.4.Thyroid nodule size has an impact on the SWE color scoringsystem.
