Objective:Through clinical case analysis,the high risk factors of multi-point CINⅢ lesions residue after conization were explored.The expression of Tafazzin protein in residual tissue was further tested,which provided a reliable clinical and experimental basis for the next step of multi-point CINⅢ.Methods:According to the postoperative pathology,159 cases of multi-point CINⅢ after cervical conization without fertility requirements,difficult follow-up,and total hysterectomy were performed,84 patients with residual disease were in the observation group,and 75 patients with no residual disease were in the control group.The correlation between age,HPV,vaginal pH,menopausal state,pregnancy,childbirth,trauma,first sexual life,number of sexual partners,education,passive smoking and multiple CINⅢ lesions after conization was analyzed.Regression analysis of high-risk factors affecting each other,and ROC curves were performed separately from individual high-risk factors,and the accuracy of predicting residual lesions was judged according to the area under the curve.Furthermore,the expression of Tafazzin protein in the residual and non-residue groups was detected by immunohistochemistry.The results were analyzed using SPSS20.0 software.Results:1.Age,vaginal pH,HPV infection,first sexual life age,number of sexual partners、passive smoking was associated with multi-point CINⅢ residual lesion after conization(P<0.05),menopausal state,pregnancy,childbirth,education,and trauma were notassociated with multiple CINⅢ postoperative residuals(P>0.05).2.Predicting the effect of residual lesions:HPV predictive effect is superior to other risk factors alone;the combined effect of the two-in-one combination is greater than the risk factors alone,and the age+HPV effect is better than the others;the six high-risk factors are combined,and the area under the curve is the largest,The forecast is the strongest.3.The expression of TAZ protein in the multi-point CINⅢ residual group after conization was significantly higher than that in the non-residue group,and the two were statistically significant(P<0.05).Conclusion:1.For patients of multi-point CINⅢ after conization with high-risk type HPV infection,if combined with age >50 years,vaginal microecological imbalance,unclean sex life history,passive smoking,can fully inform patients that the risk of residual disease is high,hysterectomy is recommended to remove residual cervical lesions,in order to prevent further progression of the disease.2.TAZ protein can be used as a predictor of multi-point CINⅢ uterine resection after conization.
