ObjectiveGastric cancer,as one of the most common malignant tumors in China,is usually diagnosed in the late stage.Chemotherapy is the main treatment. At present,as the main body of patients with advanced gastric cancer, elderly patients have decreased physical function and lower tolerance.Compared with young patients, the efficacy and safety of chemotherapy should be considered more.Traditional Chinese medicine(TCM),as a traditional medicine in China, plays a very important role in the treatment of tumors.TCM combined with chemotherapy may become a safe and effective anti-cancer program, which is the focus of current research.Brucea javanica oil emulsion injection is the first Chinese patent anticancer drug that can be injected intravenously in China. It is often used in combination with chemotherapy. The purpose of this study was to observe the efficacy and safety of Java brucea oil emulsion injection combined with S-1 in the treatment of advanced gastric cancer in the elderly, and to find an alternative treatment mode for advanced gastric cancer in the elderly.Method62 elderly patients with advanced gastric cancer admitted between May 2017 and May 2018 were retrospectively collected.31 patients treated with S-1 alone were the single drug group, and 31 patients treated with S-1 combined with brucea oil emulsion injection were the joint group.Adverse reactions, the clinical curative effect, immune function, physical condition and quality of life in two groups of patients were compared.Results1.The total effective rate of the joint group 70.97%was significantly higher than that of the single drug group 48.39%(c~2=11.889,P<0.001).2.The incidence of gastrointestinal reaction(9.68%)and peripheral neurotoxicity(3.23%)in the joint group was significantly lower than that in the single drug group(35.48%,25.81%)(P<0.05).There was no significant difference between the two groups in hepatotoxicity, nephrotoxicity and myelosuppression(P>0.05).3.There was no significant difference in Karnofsky score and QOL score between the joint group and the single drug group before treatment, and Karnofsky score and QOL score in the joint group were significantly higher than Karnofsky score in the single drug group after treatment(P<0.05).4.There was no significant difference in the immune function between the joint group and the single drug group before treatment. The serum IL-2 and IFN-?levels of the joint group after treatment were significantly higher than those of the the single drug group(P<0.05),and IL-4?IL-6 and IL-10 the levels of were significantly lower than those of the single drug group(P<0.05).Conclusion1.Compared with monotherapy with S-1,Java brucea oil emulsion injection combined with S-1 can improve the clinical efficacy for elderly patients with advanced gastric cancer.2.Compared with monotherapy with S-1,Java brucea oil emulsion injection combined with S-1 can reduce the adverse reactions caused by chemotherapy for elderly patients with advanced gastric cancer.3.Compared with monotherapy with S-1,Java brucea oil emulsion injection combined with S-1 can effectively improve the physical condition and quality of life for elderly patients with advanced gastric cancer.4.Compared with monotherapy with S-1,Java brucea oil emulsion injection combined with S-1 can enhance the immune function for elderly patients with advanced gastric cancer.
