Study On The Level Of Folate In Pregnant Women And The Influencing Factors Of Their Taking Folic Acid In Shanxi Province

Objective:There has been no study on the implementation effect of public health project of folic acid supplementation to prevent neural tube defects in Shanxi Province.The purpose of this study is to understand the red blood cell folate levels of pregnant women in Shanxi Province and the status of their taking folic acid after the implementation of this measure,so as to provide a scientific basis for further reducing the risk of local neural tube defects.Methods:This study adopts the research method of current situation investigation.The red blood cell folate levels of pregnant women in Shanxi Province and the status of their taking folic acid were investigated by multi-stage sampling in 11 cities of Shanxi Province.In the folic acid level survey,1100 subjects were recruited to participate in the questionnaire survey and blood sample collection.A total of 3850 subjects need to be recruited to participate in the survey of the status of their taking folic acid.SPSS22.0 software was used for data analysis to understand and compare the red blood cell folate level,the folic acid awareness and folic acid consumption of pregnant women in urban and rural areas of Shanxi Province.Multivariate logistic regression model and tree model were used to explore the influencing factors of folic acid consumption among pregnant women in urban and rural areas of Shanxi Province.Results:1.General information of this survey: In the folate level survey,a total of 1107 people were recruited to participate in the folate level and questionnaire survey,including 569 people in urban areas and 538 people in rural areas.In the survey of folic acid consumption among pregnant women in Shanxi Province,a total of 3850 people were recruited to participate in the survey,including 1925 in urban and 1925 in rural areas.There were significant differences in the age of pregnant women,the age of pregnant women's husband,the educational level of pregnant women,the educational level of pregnant women's husband,family per capita monthly income level,the number of pregnancies and birth history between urban and rural respondents in these two samples(P<0.05).That is to say,the demographic characteristics of urban and rural respondents in this survey are different.2.Current status of the folate levels: The results showed that the average levels of red blood cell folate in urban and rural subjects were 1001.335(656.063-1411.560)nmol/L and 993.127(658.604-1384.875)nmol/L,respectively,and the deficiency rates of red blood cell folate were 42.4% and 43.1% respectively,with no significant difference(P>0.05).3.Current status of folic acid awareness and taking folic acid: The awareness rate of folic acid in urban and rural pregnant women in Shanxi province was 97.5% and 93.5% respectively,and their rate of taking folic acid was 94.5% and 85.2% respectively.There were significant differences in the awareness rate and consumption rate of folic acid between urban and rural respondents in this study(P<0.05).The rate of standard taking folic acid of pregnant women in urban and rural areas of Shanxi Province were 14.9% and 9.6% respectively,with no significant difference(P>0.05).4.Influencing factors of folic acid consumption: Multivariate logistic regression analysis showed that the pregnant women's husband's education level,family's monthly income per capita,accidental pregnancy,whether they think it necessary to take folic acid when they are preparing for or just getting pregnant,whether they give folic acid or whether they know folic acid,these six variables have an impact on the situation of urban respondents taking folic acid(P<0.05).Pregnant women's husbands' educational level,history of childbirth,accidental pregnancy,whether they think it necessary to take folic acid when they are preparing for or just getting pregnant,whether they give folic acid and whether they know folic acid have influence on the situation of taking folic acid among rural respondents(P<0.05).Age of pregnant women,educational level of pregnant women's husband,premarital examination,pregnant women's history of abortion and accidental pregnancy,these five variables have an impact on the rates of standard taking folic acid of pregnant women in urban respondents(P<0.05).The four variables of pregnant women's husband's age,pregnant women's husband's educational level,accidental pregnancy and the source of folic acid knowledge of pregnant women had an impact on the rates of standard taking folic acid of rural respondents(P<0.05).Tree model results show that four variables,whether they think it necessary to take folic acid when they are preparing for or just getting pregnant,whether it is an accidental pregnancy,husband's education level and pregnant woman's education level,have an impact on taking folic acid of urban respondents(P<0.05).Four variables,including the history of childbirth,the distribution of folic acid,whether they think it necessary to take folic acid when they are preparing for or just getting pregnant,and women's husband's educational level,had an impact on the taking folic acid of rural respondents(P<0.05).The five variables of accidental pregnancy,the age of pregnant women's husband,premarital examination,abortion history and childbirth history had influence on the rates of standard taking folic acid of urban respondents(P<0.05).Six variables,including accidental pregnancy,premarital examination,the age of pregnant women's husband,the age of pregnant women,family's monthly income per capita and the number of pregnancies,had an impact on the rates of standard taking folic acid of rural respondents(P<0.05).Conclusion:1.The red blood cell folate level deficiency rate of pregnant women in urban and rural areas of Shanxi Province is relatively high,and some measures should be taken to reduce the deficiency rate of red blood cell folate level.2.The awareness rate of folic acid-related knowledge of pregnant women in urban and rural areas of Shanxi Province has reached the general goal of supplementing folic acid formulated by the state.3.The rate of taking folic acid of rural pregnant women in Shanxi Province and the rate of standard taking folic acid of urban and rural pregnant women did not meet the overall target of supplementing folic acid,so it is necessary to continue to strengthen the supplementary measures of folic acid.4.This study found that the influencing factors of the rate of taking folic acid and rate of standard taking folic acid of pregnant women in Shanxi Province are different,and these influencing factors are also different for each group of people.Therefore,when taking intervention measures,different methods should be adopted for different groups of people in order to achieve the best results.
