TCM Syndrome Types And Related Analysis Of Patients With Multidrug Resistant Bacteria Pulmonary Infection

Objective:To analyze the characteristics of TCM syndromes in patients with pulmonary infection caused by multidrug resistant bacteria,and to explore whether there are new TCM syndromes in clinical practice,as well as to analyze the relevant influencing factors.Method:In this study,the general clinical data of patients with multi-drug resistant bacterial pulmonary infections were recorded,summarized and processed by collecting clinical cases.Statistical analysis was completed by SPSS21.0 statistical software.The frequency description method is used for the distribution of symptoms and signs.Cluster analysis is used for the distribution of syndromes.Chi-square test or continuity correction chi-square test or Fisher's exact probability method are used for comparison between groups.Spearman correlation coefficient is used to analyze the correlation of grade data.The difference was statistically significant with P<0.05.General clinical data include(1)basic information of patients(such as name,sex,age,date of admission,number of hospitalization,etc.);(2)clinical symptoms and four diagnoses of traditional Chinese medicine;(3)complications;(4)risk factors such as invasive operation(indwelling gastric tube,indwelling urinary tube,tracheotomy,thoracic puncture,etc.);(5)sputum culture and drug sensitivity results.Result:In this study,73 patients with multi-drug resistant bacterial pulmonary infections,46 males and 27 females were collected.The TCM symptoms and signs of patients were taken as variables(no=0,with=1)for cluster analysis.The results show that TCM symptoms or signs of patients with multi-drug resistant bacterial pulmonary infection can be divided into four categories.Fisher's exact probability method was used to compare different TCM syndromes with different ages.The results showed that there was a significant difference in the occurrence probability of phlegm-heat obstruction between lung and age groups(P<0.05).The other seven TCM syndromes were compared with the age group(P>0.05),which showed that the probability of occurrence had no significant difference among the age groups.Spearman grade correlation analysis showed that there was a significant correlation between phlegm-heat obstruction of lung and age(P<0.05),and the correlation coefficient was positive(correlation coefficient=0.319>0),indicating that the incidence of phlegm-heat obstructing the lung syndrome in multi-drug-resistant bacterial pulmonary infections increases with age.Chi-square test and Fisher's exact probability method were used for statistical analysis of different TCM syndromes and different sexes(P>0.05).Here was no significant difference between sexes,indicating that there was no difference in the incidence of multiple drug-resistant bacterial pulmonary infections among different TCM syndromes.Chi-square test and Fisher's exact probability method were used to analyze the relationship between TCM syndromes and individual conditions of patients and types of combined diseases.The results showed that there was a significant difference between phlegm-heat obstruction of lung and combined diseases(P<0.05),and among the other seven TCM syndromes(P>0.05),indicating that the possibility of phlegm-heat obstruction of lung in combined diseases was significantly greater than that in other TCM syndromes.The comparison of Qi-Yin deficiency,phlegm-heat obstruction of lung and phlegm-stasis-toxin knot with patients' individual conditions was P<0.05,and that of other TCM syndromes and individual conditions was P>0.05.It shows that the possibility of three syndromes of deficiency of both Qi and Yin,phlegm-heat obstruction of lung and phlegm-stasis toxin in patients' personal conditions is significantly higher than that of other syndromes.Fisher's exact probability method was used to analyze the relationship between TCM syndromes and invasive operation.The results showed that there was no correlation between multi-drug resistant bacterial pulmonary infection and invasive operation by comparing the TCM syndromes with invasive operation group(P>0.05).Conclusion:(1)The TCM syndromes of multi-drug-resistant bacterial pulmonary infection are not perfect enough.According to the Diagnostic Basis of Wind Warming Lung Fever in the Criteria for Diagnosis and Curative Effect of Diseases and Syndromes of Traditional Chinese Medicine promulgated by the State Administration of Traditional Chinese Medicine,Wind Warming Lung Fever can be divided into six syndromes: wind-heat invading the lung,phlegm-heat obstructing the lung,excessive heat in the lung and stomach,heat closing pericardium,deficiency of both Qi and Yin,and positive extrication.TCM symptoms and signs of pulmonary infection caused by multi-drug resistant bacteria can not be covered.Based on the analysis of the included samples,two new syndromes of phlegm and blood stasis,toxin stagnation and yin-yang deficiency were concluded in this study.(2)TCM syndromes of multi-drug-resistant bacterial pulmonary infection are phlegm-stasis-toxin stagnation,phlegm-heat obstruction of lung and deficiency of both qi and yin.(3)The type of phlegm-heat obstructing the lung of multi-drug-resistant bacterial pulmonary infection was positively correlated with age,while other syndromes were not correlated with age.(4)TCM Syndrome Types of Multidrug Resistant Bacteria Pulmonary Infection have nothing to do with gender.(5)The possibility of phlegm-heat obstructing lung syndrome in the combined diseases of multi-drug-resistant bacterial pulmonary infection is significantly higher than the other seven TCM syndromes.(6)The possibility of Qi-Yin deficiency,phlegm-heat obstruction of lung and phlegm-stasis-toxin stagnation in the individual situation of patients with multi-drug-resistant bacterial pulmonary infection is high,while the possibility of other syndromes is low.(7)Multidrug-resistant bacterial pulmonary infections were not associated with invasive procedures.
