Background:Stroke is a multifactorial disease with high morbidity and mortality.With the changing of people's life,aging of the population and living standard improving,the morbidity of stroke is rising year by year.Early assessment of the patients' prognosis is necessary for the rational treatment options.Therefore,finding effective plasma markers is very important.Objective:The objective of this study is to observe the association between uricemia,homocysteine,D-Dimer,fibrinogen and prognosis of acute cerebral infarction,and to explore the possible mechanism and to provide a theoretical basis for the prognosis.Methods:Consecutive patients(n=156)presenting with acute cerebral infarction were included in the study by strict inclusion and exclusion in the Xin Hua Hospital Affiliated to Shanghai Jiao Tong University School of Medicine from January 2013 to July 2015.The patients will be grouped according to severity on admission and prognosis evaluated with the modified Rankin score grading.We used SPSS16.0 software for statistical analysis,compared different clinical outcome of patients with acute ischemic stroke differences in baseline characteristics and used Logistic regression model to analyze related factors.Results:1.There were 100 patients in the good prognosis group(64.10%)and 56 patients in the poor prognosis group(38.89%).2.The mild group is 74 cases(47.43%)and the severe group is 82 cases(52.56%).Hypertension(P=0.001),homocysteine(p=0.01),fibrinogen(P=0.01)is associated with the severity of acute cerebral infarction on admission.3.The NIHSS scores on admission(P<0.001),homocysteine(P=0.01)and D-Dimer(P=0.001)in the poor prognosis group were significantly higher than those in the good prognosis group,while the SUA levels in the poor prognosis(P<0.001)were significantly lower than those in the good prognosis group.4.A logistic regression adjusted for confounders confirmed that SUA and the prognosis in patients with stroke were positively correlated(OR=1.015,P=0.01,95%CI),besides,hcy level(OR=0.55,P=0.015,95%CI)and the NIHSS scores(OR=0.881,P=0.005,95%CI)on admission were independently related with the prognosis of the patients.Conclusions:1.HBP,homocysteine,fibrinogen were positively correlated with the severity of acute cerebral infarction on admission.2.The NIHSS scores on admission and homocysteine were independent factors for acute cerebral infarction with poor prognosis,while uric acid level was an independent protective factor of acute cerebral infarction.3.D-Dimer may interact with other factors for poor prognosis of acute cerebral infarction at the 90 th day.
