BACKGROUND: With the development and clinical application of ultrasonic technology,detection rate of thyroid nodule increased rapidly,and also thyroid cancer.Although malignant degree of thyroid cancer is low,but the elderly thyroid cancer lymph node metastasis rate and the mortality rate are extremely high.This study hopes to analyze the thyroid carcinoma lymph node metastasis conditions and prognostic factors,and conclude thyroid carcinoma surgery patterns and how to prevent and deal with postoperative hypocalcemia in elderly patients.METHODS: Retrospectively analyzed 743 elderly patients who underwent thyroid cancer surgery in Head and Neck apartment of Renji Hospital from March 2002 to July 2015,recorded including general situations,surgical patterns,pathological findings,preoperative postoperative serum calciums,parathyroid hormone concentrations and follow-ups.RESULTS: Males,multifocal thyroid cancer were prone to cervical lymph node metastasis(P < 0.05).Males,lymph node metastasis patients were prone to recurrence(P* < 0.05).Morover,elderly patients underwent lymph node dissections achived decreased serum calcium concentration more easily(P **<0.05).CONCLUSIONS: Elderly patients with thyroid carcinoma are easily to occur lymph node metastasis,therefore,prophylactic central neck lymph node dissection should be done.Due to lymph node dissection increasing the probability of postoperative hypocalcemia,surgeons should strengthen the identification and protection of parathyroid,and the detection of postoperative serum calcium.If hypocalcemia occurrs,approachs should be timely taken to reduce serious consequences of hypocalcemia.
