Diagnosis And Prognosis Evaluation Of 3D-CTA For Responsible Aneurysm Of Spontaneous Subarachnoid Hemorrhage

Objective:To investigate the value of 3D-CTA in the diagnosis of spontaneous subarachnoid hemorrhage aneurysm and the evaluation of Fishier classification in the prognosis of patients with subarachnoid hemorrhage.Methods:Retrospective analysis of 76 cases of aortic aneurysm diagnosed by 3D-CTA and DSA by CT scan in the Inner Mongolia Autonomous Region People's Hospital from March2016 to March 2019,with surgical pathology The detection rate of both checks was verified as a gold standard.(1)Comparison of CTA and contrast DSA detection methods for intracranial aneurysms;(2)evaluation of Fisher classification and GOS score;(3)analysis of patients with poor prognosis of subarachnoid hemorrhage Good hair.Result:71 patients with intracranial aneurysm diagnosed by surgery,a total of 79 aneurysms;64 patients with intracranial aneurysms detected by CTA,a total of 67 aneurysms;67 cases of intracranial aneurysms detected by DSA There are 69 aneurysms.The positive rate,sensitivity,specificity and accuracy of CTA detection were 86.1%,90.5%,80.0%and 89.9%,respectively.The positive rate,sensitivity,specificity and accuracy of DSA detection were88.6%and 93.2%,80.0%,and 92.4%,after statistical analysis,the difference between the two methods was not statistically significant.G=-0.879,P=0.000<0.05,the patient's bleeding volume,bleeding location and the possibility of cerebral vasospasm after hemorrhage were negatively correlated with the patient's prognosis,ie the GOS score decreased with increasing Fishier grading.Patients with subarachnoid hemorrhage caused by ruptured middle aortic aneurysm were more likely to develop CVS than the other intracranial arteries,and the prognosis was poor(?~2=11.213,p=0.047<0.05).Conclusion:CTA can be used as a routine examination for the detection of intracranial aneurysms;Fishier classification can well judge the possibility of cerebral vasospasm from the perspective of imaging,so as to judge the prognosis of patients,and to formulate the next surgical plan for the clinic.The diagnosis and treatment of patients and the provision of more complete imaging data,the evaluation of the prognosis of patients has extremely important significance;the prognosis of bleeding caused by rupture of middle cerebral aneurysm is poor,should be highly regarded clinically.
