Objective: To reconstruct the model of intestinal fistula and intestinal tract by 3D modeling technology,and to clarify whether 3D modeling technology is helpful for the diagnosis and treatment of fistula type,location and number.Methods: From January 2016 to January 2019,44 patients admitted to the Department of Gastrointestinal Surgery of the First Affiliated Hospital of Hunan Normal University were definitely diagnosed as intestinal fistula.Among them,30 were males,accounting for 68.2% and 14 were females,accounting for 31.8%.The youngest age was 34 years old and the oldest was76 years old,with an average age of 55.7 years.They had duodenal,colon and small intestinal fistula respectively.From January 2016 to November2017,32 patients with intestinal fistula were divided into group A and group B.From December 2017 to January 2019,12 patients with intestinal fistula were treated with 3D modeling.There was no significant difference in general condition between the two groups.Combining with the data of angiography and CT,the author reconstructed the 3D model of the two-dimensional CT image by using Mimics 17.0.Through the observation of the 3D model,the situation of intestinal fistula was more thoroughly understood,the location,type and size of the fistula were defined,and the direction of the fistula was understood.The continuity of the intestine was observed to determine whether there was obstruction in the distal part of thefistula.The general data and condition of the two groups were collected and analyzed concretely.The recovery status,the time from diagnosis to decision of operation,the length of hospitalization,the monitoring of white blood cells,C-reactive protein and procalcitonin(PCT)were compared between the two groups.The collection of laboratory data was carried out on the 1st,4th and 7th day after treatment.Inflammation indexes of the two groups were compared and analyzed to determine the changes.Condition.Patients need to undergo angiography during treatment,so compare the application of 3D modeling technology in the treatment of intestinal fistula,whether it can effectively reduce the number of angiograms during treatment,play a protective role for patients,and avoid radiation damage to the greatest extent.During the treatment period,patients with sepsis need to record,determine the number of attacks,and compare whether 3D modeling technology can shorten the treatment cycle and reduce the incidence of sepsis.Result: 32 patients in group A were treated by routine imaging examination.After drainage,infection control and nutritional support,26 of them recovered spontaneously and 6 of them were cured after operation.Twelve patients in group B were treated with 3-D model.After treatment of drainage,infection control and nutritional support,10 of them recovered spontaneously and 2 of them were cured after operation.There was no significant difference between the two groups in the time spent in deciding to undergo surgical treatment after diagnosis(p > 0.05).There was no significant difference in hospitalization time between the two groups(p >0.05).There was no significant difference in white blood cells between the two groups on the first day,the fourth day and the seventh day(p > 0.05).There was significant difference in C-reactive protein between the two groups on the first day,the fourth day and the seventh day after treatment(p< 0.05).There was no significant difference in PCT between the two groups on the first day,the fourth day and the seventh day(p > 0.05).There was no significant difference in sepsis between the two groups(p > 0.05).There was a significant difference in the number of angiograms between the two groups(p < 0.05).CONCLUSION:(1)3D modeling technology is superior to the conventional treatment group in reducing C-reactive protein by modeling the location of intestinal fistula and drainage tube,and adjusting the location of drainage tube and fistula through the established 3D model,which suggests that it is helpful for full drainage.(2)3D modeling of intestinal fistula patients can effectively reduce the number of angiograms during treatment.
