Study On Self-management Level And Influencing Factors Of Patients With Obstructive Sleep Apnea Hypopnea Syndrome

ObjectiveTo investigate the status quo of self-management in patients with obstructive sleep apnea hypopnea syndrome(OSAHS),to analyze the influencing factors of self-management level in OSAHS patients,and to provide data reference and decision-making basis for health guidance and self-management of OSAHS patients.MethodsThis study adopts the quantitative research method which was a cross-sectional survey and carried out from January 2018 to September2018.297 subjects who were diagnosed with OSAHS were recruited by random cluster sampling method from 2 hospitals which were randomly selected from third-grade class-A hospitals in Changsha.Research instruments included five parts: General Information Questionnaire,OSAHS self-management scale,Pittsburgh sleep quality index and Epworth sleeping scale.All data were analyzed by SPSS24.0 statistical software.The statistical methods included descriptive analysis,t test,rank sum test,Spearman correlation analysis and multiple linear regression analysis.Results(1)Among 297 OSAHS patients,the total median and interquartile spacing of self-management was 59(17)points,with 8 patients with good self-management level,accounting for 2.69%;100 patients with medium level,accounting for 33.67%;189 patients with general level,accounting for 63.64%.Scores of each dimension of self-management questionnaire:life management,treatment management,knowledge and skills management,and psychological management scores with median and quartile spacing of 28(11),15(5),7(4)and 8(3)points respectively.(2)OSAHS patients' scores of sleep quality and daytime sleepiness:(1)The scores of the Pittsburgh sleep quality index scale of OSAHS patients ranged from 2 to 21,with an average score of 7.55±3.25.Among the 297 patients,195 patients had sleep disorder,accounting for65.66%,and 102 patients without sleep disorder,accounting for34.34%.(2)The mean and standard deviation of the total score of the Epworth Sleepiness Scale in OSAHS patients were(12.04 ±4.83).Among297 OSAHS patients,90(30.30%)were non-narcolepsy,160(53.87%)were mild narcolepsy and moderate narcolepsy,and 47(15.83%)were severe narcolepsy.(3)the results of univariate analysis of OSAHS patients' self-management level show that different marital status,long-term residence,type of household income,educational level,occupation,payment method,and apnea hypoventilation index,the difference of self-management behavior score was statistically significant(P<0.05),the statistics(t?F?H)are respectively-2.578,11.836,96.728,59.284,89.772,111.666,4.714,79.111.(4)correlation analysis results of OSAHS patients' self-management level show that the correlation coefficient between the total score of self-management and the total score of sleep quality in OSAHS patients was-0.058(P>0.05),and the correlation coefficient was not statistically significant.,and was negatively correlated with the total score of Epworth sleepiness scale(rs=-0.367,P<0.05).(5)results of multivariate linear regression analysis of OSAHS patients' self-management level,a total of 5 factors were included in the regression model,namely the Epworth Sleeping Scale,long-term residence,average monthly household income,education level,and medical payment method.The correlation coefficient R of the regression model is 0.803,and the determination coefficient R2 is 0.645.Through F test,F=65.548,P<0.01,the fitted multiple linear regression equation has statistical significance.Conclusions(1)The self-management level of OSAHS patients is in the middle and lower.(2)The influencing factors of OSAHS patients' self-management level mainly include daytime sleepiness,long-term residence,average monthly household income,professional types,education level and medical payment mode.
