Objective:We examined and analyzed the value of serum monosaccharide concentration in the diagnosis of trigeminal neuralgia and its relationship with gender and etiology by detecting the concentration of monosaccharide in serum of healthy people and patients with trigeminal neuralgia. Methods:We collected 132 sera including 66 patients with trigeminal neuralgia and 66 healthy subjects.Patients with trigeminal neuralgia were treated at the Affiliated Hospital of Qingdao University from April 2017 to May 2018.The health examiners were medical examiners who were examined at the physical examination center of the Affiliated Hospital of Qingdao University hospital.First,we were divided into two groups according to the etiology of patients with trigeminal neuralgia,11 patients with post-herpetic trigeminal neuralgia, and 55 patients with primary trigeminal neuralgia. Then we divided the patients into two groups according to the gender of the patient(n=29)and female(n=37).We carried out HPLC analysis on the derivatized monosaccharide standard to obtain the liquid phase spectrum of the monosaccharide standard, and then plotted the standard curve of the monosaccharide standard according to the peak area and the corresponding concentration to obtain a linear regression equation. We obtained the liquid chromatogram of the sample by direct derivatization and post-derivative derivatization of the sample, and the peak area of the sample was substituted into the standard curve equation to obtain the concentration of each monosaccharide in the sample, and the t-test was used to determine each Statistical differences between the groups, and the ROC curve to evaluate the value of serum monosaccharide concentration in the diagnosis of trigeminal neuralgia.Results:(1)There was no significant difference in the basic data between patients with trigeminal neuralgia and healthy subjects(P>0.05).(2)The concentration of free mannose(Mannose,Man)in the serum of the trigeminal neuralgia group was higher than that of healthy subjects. The difference between the two groups was statistically significant(P<0.05),and the area under the curve was 0.63.The concentration of free glucose(Glucose,Glc)in serum was lower than that of healthy subjects. There was a statistical difference between the two(P<0.05),and the area under the curve was 0.67.(3)The concentrations of free Man and Glc in the patients with post-herpetic trigeminal neuralgia were higher than those in patients with primary trigeminal neuralgia, and the difference was statistically significant(both P<0.05).(4)There were statistical differences between the concentrations of seven monosaccharides in the serum of the trigeminal neuralgia group(P<0.05).They were mannose, galactosamine(Galactosamine,Gal N),glucose, and glucose aldehyde.Acid(Glucuronic acid,Glc A),galactose(Gal),xylose(Xylose,Xyl)and fucose(Fucose,Fuc).Among them, the concentration of Man,Gal and Fuc increased compared with those of healthy subjects, while the concentrations of Gal N,Glc A,Glc and Xyl decreased compared with those of healthy subjects. The areas under the Man,Gal N,Glc A,Glc,Gal,Xyl,and Fuc curves were 0.81,0.68,0.70,0.72,0.73,0.66,and 0.81,respectively.(5)The concentrations of Man,Glc N,Gal N,Glc A and Glc in the serum of patients with trigeminal neuralgia were different between male and female patients, and the difference was statistically significant(both P<0.05).Conclusion: We found a statistically significant difference between the two free monosaccharide concentrations in patients with trigeminal neuralgia and healthy subjects. We also found that the concentration of seven degraded monosaccharides in serum was statistically different from that of healthy subjects, indicating that it has a good significance for the diagnosis of trigeminal neuralgia. After our analysis,we found that some monosaccharide concentrations in patients with trigeminal neuralgia of different etiology and gender are different. Therefore,we can help to diagnose the treatment of trigeminal neuralgia and the differentiation of the causes and gender differences in patients with trigeminal neuralgia by detecting the level of monosaccharide in serum.
