Objective:A retrospective analysis of clinical cases was conducted to investigate the relationship between chorioamnionitis and intrauterine infectious pneumonia in preterm infants less than 34 weeks old.Methods :We selected 2015.10-2017.10 in our hospital for obstetrics and gave birth to placenta pathological examination,and transferred to our hospital's NICU <34weeks premature infants as a study object(362 cases),according to placental pathological results,divided into chorionic amnion negative,fluff Membrane amnion positive 2groups,compared with the number of cases of intrauterine infectious pneumonia in premature infants in each group,to assess whether there is a correlation between the two;if there is correlation,the mother placenta pathological examination is positive,and transferred to the NICU Preterm infants with less than 34 weeks of diagnosis of intrauterine infectious pneumonia were included in the study.A total of 103 subjects were included.According to the grading and staging criteria of chorioamnionitis,they were divided into three groups: early group,middle group,late group and I group,II group and III group.Compare the general conditions,critical scores,highest oxygen uptake scores among the early,middle,and late groups,and between the I,II,and III groups,the number of comorbidities,respiratory support time,and Differences in treatment,etc.,to correct the differences in growth and development indicators of premature infants at 6months of age.Result:1.Comparison of general conditions: number of premature rupture of maternal membranes,number of prebiotics used before birth,number of prenatal fever,number of prenatal mother white blood cells,prenatal mother C-reactive protein,gestational age of children,birth weight of children The comparison was statistically significant(P<0.05);2.Clinical comparison: respiratory distress score,critical score,invasive ventilation time and non-invasive ventilation time,number of complications,length of hospital stay,number of deaths were statistically significant(P<0.05);3.Blood gas analysis and comparison of infection indicators: the highest oxygenuptake rate,leukocyte and procalcitonin in the 1 hour after admission were statistically significant(P<0.05);4.The comparison of growth and development in the follow-up period of 6 months:the number of rehospitalization,the number of abnormalities in 20 neurological examinations,the difference of MDI and PDI were statistically significant(P<0.05).Conclusions : Chorioamnionitis has a clinical impact on intrauterine infectious pneumonia in preterm infants.1.The deeper the infiltration of placenta,the more severe the inflammation,the more severe the risk of pneumonia in premature infants.The more cases of complications,the longer the period of respiratory support during treatment,the higher the oxygen concentration,the more hospital stays More,the worse the prognosis of the child;2.The deeper the infiltration of placenta,the more severe the inflammation,the higher the infection index of premature infants such as white blood cell count and procalcitonin,the heavier the infection;3.The deeper the degree of infiltration of placenta,the more severe the inflammation,the lower the developmental scale of infants and young children in the premature infants.The more abnormal cases of 20 neurological examinations in newborns,the worse the neurodevelopmental prognosis of children.
