Objective:The purpose of this study was to investigate the relationship between the age at natural menopause(ANM)of women and the coronary Gensini score,and to identify risk factors for coronary artery disease in women.Methods : We collected natural postmenopausal women who underwent coronary angiography in Qingdao municipal hospital and affiliated hospital of Qingdao university from October 2017 to October 2018.A total of 221 subjects were identified according to inclusion criteria and exclusion criteria.According to ANM,they were divided into 3 groups,ANM<46 years old group(38 cases),46 years old ?ANM ?52 years old group(149 cases)and ANM>52 years old group(34 cases).We collected the clinical data of the study subjects,consulted the relevant medical records,recorded baseline data and biochemical indicators such as blood lipids and fasting blood glucose,and calculated the Gensini score based on the results of coronary angiography.Baseline data,biochemical indicators,and Gensini scores were compared between the three groups.Statistical analysis was performed using SPSS22.0 pairs.The measurement data were expressed as mean ± standard deviation(X±S),and the count data was expressed in terms of number and percentage.The comparison of the mean of the three groups of samples was analyzed by ANOVA.The least significant difference test(LSD)was used for the comparison between the two groups.The comparison of the count data rates was performed by the ?2 test.Pearson correlation analysis was used to analyze the correlation between relevant indicators and coronary Gensini score.Risk factors for coronary artery lesions were studied using multiple linear regression analysis.Results : 1.There was a statistical difference in the mean age between the three groups(P<0.01).Pairwise comparison showed that the mean age of patients in the ANM<46 years old group(61.74±7.02 years old)was significantly lower than that in the 46?ANM?52 years old group(66.77±7.04 years old)and the ANM>52 years old group(67.53±5.12 years old)(P<0.01);There was no significant difference in the mean age between the 46?ANM?52years old group and the ANM>52 years old group(P>0.05).The mean menopause years between the three groups were statistically different(P<0.01),pairwise comparison showed that the mean menopause years between the ANM<46 years old group(17.34±6.74 years)and the 46?ANM?52 years old group(16.67±7.31 years)showed no statistical difference(P>0.05),and they were greater than the ANM>52 years group(12.76±4.90 years)(P<0.01).2.There was no significant difference in family history of coronary heart disease,hypertension and diabetes between the three groups(P>0.05).3.There were no significant differences in blood lipid,fasting blood glucose,serum creatinine,uric acid and fibrinogen between the three groups(P>0.05).4.There was a significant difference in the Gensini score among the three groups(P<0.05).Pairwise comparison of the three groups showed that the average Gensini score in the ANM<46 years old group(39.37±24.18)was significantly greater than the other groups,and there was no significant difference in the mean Gensini score between the 46?ANM?52years old group(28.17±22.96)and the ANM>52 years old group(28.56±21.43)(P>0.05).It suggested that compared with patients aged 46 years with ANM?46,patients with ANM<46years old had higher Gensini score and more severe coronary artery lesions.5.Pearson correlation analysis was conducted to study the correlation between various indexes and Gensini score,and the results showed that ANM and HDL-C were negatively correlated with Gensini score;Menopause years,TG,TC,LDL-C,uric acid and FIB were positively correlated with Gensini score.6.Multiple linear regression models of the risk factors for Gensini coronary score showed that LDL-C and FIB were positively correlated with coronary Gensini score,while ANM and HDL-C were negatively correlated with coronary Gensini score..Conclusions:1.The Gensini score of females with ANM<46 years old were significantly higher than those of females with ANM? 46 years old,and the incidence of coronary artery disease was younger in women with ANM<46 years old.2.LDL-C and FIB are independent risk factors for coronary artery Gensini score,while HDL-C is a protective factor for Gensini score.3.In women,ANM was negatively correlated with the Gensini score of coronary artery.Early ANM was a risk factor for coronary artery lesions,and early menopause was associated with the occurrence and development of coronary artery lesions.
