ObjectiveWe choose patients with chronic cerebra lvascular occlusion confirmed by angiography,review the results of craniocerebral MRI and DSA in patients,with the help of quantitative scoring of white matter lesions and collateral circulation grading,we retrospectively analyzed the relationship between ischemic white matter lesions and collateral circulation formation in patients with chronic cerebra lvascular occlusion,in order to clarify what factors may be associated with white matter lesions in such patients.MethodInpatients admitted to the neurology department of the yantai yuhungding hospital from January 2010 to December 2015 were selected,all patients had chronic cerebra lvascular occlusio,during hospitalization,all patients underwent craniocerebral MRI and DSA examinations.They all met the screening criteria we set,review the results of craniocerebral MRI and DSA of them,gender,age,medical history,laboratory examination and imaging examination of these patients were collected.NHISS score and mRS score at admission and discharge were recorded.They were assessed with white matter lesion score(Fazekas scale,modified Scheltens scale)and collateral circulation,Count the number of stenosis vessels,to explore the relationship between ischemic white matter lesions and collateral circulation formation by statistical methods,to determine what factors may be associated with white matter lesions in such patients.ResultA total of 117 patients were enrolled,including 79 males and 38 females.1.For patients with chronic cerebra lvascular occlusion,The severity of white matter lesion may not be related to the degree of collateral circulation opening and collateral circulation type.2.Univariate logistic regression analysis was used to analyze the factors affecting Fazekas score.It was found that the maximum pulse pressure difference and the number of severe cerebral vascular stenosis were correlated with Fazekas score.Multivariate logistic regression analysis showed that the number of severe cerebral vascular stenosis was still correlated with Fazekas score.3.Univariate logistic regression analysis was used to analyze the factors affecting Scheltens score.It was found that the number of severe cerebral vascular stenosis and Fazekas score were correlated with the score.Univariate analysis(P < 0.1)was included in multivariate logistic regression analysis,and there were no correlative variables with Scheltens score.4.Univariate logistic regression analysis showed that age,high density lipoprotein,platelet,glutamic oxaloacetic transaminase and Fazekas score were correlated with adverse prognosis.Multivariate analysis showed that age,glutamic oxaloacetic transaminase and Fazekas score were still correlated with adverse prognosis.Conclusion1.For patients with chronic cerebra lvascular occlusion,The severity of white matter lesion may not be related to the degree of collateral circulation opening and collateral circulation type.2.The number of severe cerebral vascular stenosis was correlated with the severity of white matter lesions(Fazekas score)in patients with cerebral vascular occlusion.3.Age,glutamic oxaloacetic transaminase and Fazekas score were associated with poor prognosis in patients with cerebral vascular occlusion.
