Objective:In order to guide the clinical practice,a retrospective study was conducted to investigate the safety and efficacy of the LVIS stent device in the treatment of intracranial microaneurysms(diameter of aneurysms ? 3 mm). Methods:The “Lianzhong” medical record system of the Affiliated Hospital of Qingdao University was adopted, and a number of 35 patients with microaneurysm information from the Department of Neurosurgery(Shinan Campus and Lushan Campus)of the Affiliated Hospital from January 2016 to December 2018 were collected with strict inclusion criteria and exclusion criteria.LVIS stent assisted the efficacy of endovascular treatment was employed to conduct retrospective analysis, and immediate aneurysm embolization rate, surgical complications were investigated, postoperative follow-up digital subtraction angiography(DSA)was used to confirm the results and evaluate the prognosis GOS score.Twenty-nine cases of ruptured aneurysm were selected as LVIS group and 25 cases of aneurysms with Enterprise stent assisted embolization of small aneurysms in the published article were selected as the Enterprise group, the immediate aneurysm embolization rate of the two groups were compared. The data of two groups were input into SPSS19.0 for statistical analysis, and the correct statistical method was employed with P value <0.05 as statistical significance.Results:Thirty-five aneurysms and no intraoperative aneurysm rupture were shown in this study, immediate digital subtraction angiography(DSA)displayed the final result were 24 Raymond I embolizations,10 Raymond II embolizations, and 1 Raymond III embolization. One case of thromboembolic thrombosis occurred in the stent and the thrombus was dissolved by injecting urokinase into the microcatheter. One case of delayed thrombosis occurred after surgery and the urokinase thrombolysis was injected through the microcatheter. After 6 months of clinical follow-up,digital subtraction angiography(DSA)showed that the results were 31 Raymond I embolizations,3Raymond II embolizations,1 Raymond III embolization, and(GOS score 4-5)33 cases were shown excellent effect.During the follow-up,the LVIS group showed complete embolization in 25 cases, subtotal embolization in 3 cases, partial embolization in one case, while the Enterprise group showed complete occlusion in 22 cases, subtotalocclusion in 3 cases, and the two groups date of P=0.845,which suggested LVIS group and Enterprise group were no significant difference in the rate of contrast embolization between the follow-up.There were 2 cases of surgery-related complications in the LVIS group and no death was occurred. There were 3 cases of surgery-related complications in the Enterprise group, and 1 case of death. The incidence of surgical complications was compared between the LVIS group and the Enterprise group,p=0.862,suggested that LVIS group and Enterprise were no significant differences in the complications associated with surgery. The prognostic evaluation of the LVS stent group was as follows: 20 in 5,4in 4,and 2 in 3 and the GOS score in the Enterprise stent group was 16 in 5 There were 6cases in 4 points,2 cases in 3 points, and 1 case in 2 points. We had a GOS score of 4-5 as a good prognosis, and a score of 4 or less as a poor prognosis. The prognostic GOS scores of the LVIS group and the Enterprise group were compared,P=0.519,suggested that there was no significant difference in prognosis between the LVIS group and the Enterprise group. Conclusions:The LVIS stent device, which as a method for the interventional treatment of intracranial microaneurysms was safe and effective.Nevertheless,it is necessary to be careful to alert and prevent complications of the surgical thrombus event.
