Objectives To explore the prevalence and component related factors of metabolic syndrome(MS)among adults in Hebei Province,and to determine the best indicators for prediction and diagnosis of MS,so as to provide the rationale for developing corresponding comprehensive prevention and control measures for MS in Hebei Province.Methods Eight thousands one hundred and sixty two(8162)adults over 18 years old were selected by multistage random cluster sampling method from 13 monitoring sites in Hebei Province from October to December in 2015 for questionnairing,physical examinations and laboratory tests.SPSS 22.0 and SAS 9.2 statistical software were used to collate and describe the data.Amos 21.0 was used to construct the six factors models of MS,and confirmatory factor analysis was used to fit and evaluate the populations of different gender,age and region,thus obtaining suitable component factors models for the respective populations.ROC curve was used to evaluate the ability of each factor index to predict and diagnose MS.Results 1 The prevalence of MS among adults in Hebei Province was 27.08%in 2015.Specifically,the prevalences of males and females were 25.43%and 28.64%(?~2=1.017,P=0.313)respectively,that among the young,middle-aged and elderly populations were17.98%,37.06%and 41.00%(?~2=79.494,P<0.001)respectively,and that in the urban and rural areas were 28.62%and 25.98%respectively(?~2=0.864,P=0.353).2 The prevalences of high triglyceride(TG)and abnormal blood pressure in males were higher than those of females(30.66%VS 21.63%and 57.61%VS 73.79 respectively).The prevalences of central obesity and low high density lipoprotein cholesterol(HDL-C)in females were higher than those of males(57.65 VS 40.54 and 44.10 VS 31.21 respectively).The prevalences of pathoglycemia and dysarteriotony increased with age,and the prevalences of central obesity and high-TG increased with age too and decresaed a little after peaking.The prevalence of low HDL-C in urban areas was higher than that in rural areas(40.43%VS 36.01%),and the prevalence of abnormal blood pressure was lower than that in rural areas(45.89%VS 53.79%).3 The results of different combinations of MS components showed that except the prevalence of central obesity+pathoglycemia+dysarteriotony was highest in the elderly population,the prevalence of the combination of central obesity+high TG+dysarteriotony was higest in the rest populations,and the prevalence of the the combination of five components was lowest in all populations.4 The results of clustering of MS components showed that there were 0,1,2,3,4 or 6 kinds of abnormal items aggregated in the MS populaiton,and their proportion were 13.95%,28.41%,26.43%,19.97%,9.24%and 2.00%respectively.When the prevalence of MS reached peak in different groups,it gradually decreased with the abnormal component items increased.5 Multivariate analysis showed that model 3 had a better fittness to different populations of gender,age and region.Mean arterial pressure(MAP),TG/HDL-C,fasting blood glucose(FPG)and waist-to-height ratio(WHtR)were the main factors for the adult population in Hebei Province.By analyzing the six models constructed by Amos21.0,we found that the factor loads of waist circumference(WC),body mass index(BMI),WHtR and TG/HDL-C which could reflect obesity and lipids contents were higher in different groups(>0.5).6 The factor load of BMI for males ranged from 0.73 to 0.86 and overweight/obesity could explain the main characteristics of the model.There was a covariant relationship between obesity degree and lipids level and the factor load was 0.56(>0.5).The factor loads of WC and WHtR for women ranged from 0.59 to 0.85,and central obesity could explain the main characteristics of the model.In the young poputaion,indicators other than FPG(<0.5)were highly correlated with MS.The factor loads of WC and WHtR indicators in urban population ranged from 0.58 to 0.85,and central obesity could explain the main characteristics of the model.7 ROC curve analysis results show that:The area under ROC curve of WHtR washighest,which was 0.853(95%CI:0.844~0.861,P<0.01).Conclusions 1 The prevalence of MS of adults in Hebei Province increased with age.Especially,the prevalence of males shows an inverted"U"pattern with the increasing of age.2 Among all components of MS in adult population in Hebe Province,the prevalence of abnormal blood pressure ranks highest.In addition,the prevalence of each components were different in different groups.It is suggested that primary professionals should strengthen the health management of hypertension,appropriate prevention and control measures should be taken according to the prevalence of different populations.3 Central obesity+high TG+abnormal blood pressure is the main combination of MS in Hebei Province.The clustering of 1~2 abnormal components is most common in MS.4 The fitness of single-factor model is better than that of other models,and MAP,TG/HDL-C,FPG and WHtR are the four factors that could provide the best explanations associated with MS components.Overweight/obesity,central obesity and dyslipidemia are the key to reduce the incidence of MS in Hebei Province.Compared with other factors,WHtR has a better predictive and diagnostic value
