ObjectivesTo investigate the long-term impact of hypertension in pregnancy(HIP)on maternal estimated glomerular filtration rate(eGFR)by analyzing the gestation and health examination data of Kailuan Group female wokers.MethodsA total of 4243 females in Kailuan Group were enrolled in the cohort,who gave birth from October 1976 to June 2014 and participated the annual examinations in2006,2008,2010,2012,2014 in Kailuan Medical Group with questionnaires,physical examination and laboratory examination(serum creatinine,fasting blood glucose,high density lipoprotein cholesterol,and low density lipoprotein cholesterol data etc.).The inpatient medical records of the Kailuan Group hospitals and family planning files were recorded the delivery information of females.The subjects were devided into HIP group(n=620)and NHIP group(n=3623)according to the history of HIP,then divided to three subgroups according to the postpartum time(<5,5-15,?15 years).The eGFR was assessed by the equations from Chronic Kidney Disease-Epidemiology Collaboration(CKD-EPI).The difference of eGFR between HIP and NHIP groups were observed,and the impact of HIP on eGFR was analyzed by Logistic regression models.ResultsThe mean follow-up time was(16.97±8.88)years.There was no significant difference in the eGFR or the proportion of eGFR>125 mL/(min·1.73m~2)or eGFR<60mL/(min·1.73m~2)between the HIP and NHIP groups,whereas the proportion of eGFR>125mL/(min·1.73m~2)in the HIP group was 20.9%(n=29),higher than 14.3%(n=167)in the NHIP group in 5-15 years postpartum(P<0.05).Logistic regression models revealed that after adjustment for confounding factors,the risk of eGFR>125 mL/(min·1.73m~2)in HIP group was 1.70 times as high as NHIP group in the 5-15 years postpartum[OR(95%CI)was 1.70(1.01~2.87)].Conclusions HIP is an independent risk factor for high eGFR in 5-15 years postpartum.Figure 0;Table 8;Reference 34...
