ObjectivesTo compare the clinical effects of holmium laser lithotripsy(FURL)under flexible ureteroscope in the treatment of renal calculi with diameter less than 1.5 cm and1.5-2.0 cm.MethodsA retrospective analysis was made of 178 patients with renal calculi admitted to our urology department from January 2017 to June 2018.All of them underwent FURL operation.121 patients in group A(diameter<1.5cm)and 57 patients in group B(1.5cm<diameter<2.0cm)were treated with FURL.The average operation time,hospital stay,success rate of operation and stone clearance rate(SFR)in one month after operation were observed and compared between the two groups.To evaluate the clinical effect of FURL in the treatment of renal calculi between 1.5 cm in diameter and 1.5-2.0 cm in diameter,the differences of SFR,Hb,WBC and complications were analyzed.Results1 The success rate of operation in group A was 99.2%(120/121)and that in group B was 96.5%(55/57).There was no significant difference in the success rate between the two groups(P=0.241).2 The SFR of inferior calyx was 81.5%in group A and 69.2%in group B.There was no significant difference in SFR between the two groups(P>0.05).3 The average decrease value of hemoglobin in group A was(4.42±1.09)g/L and that in group B was(4.74±1.14)g/L.There was no significant difference in the decrease value of hemoglobin between the two groups(P>0.05).4 The average increase of WBC in group A was(2.90±1.02)×10~9/L and(3.09±0.96)×10~9/L in group B.There was no significant difference in the increase of WBC between the two groups(P>0.05).5 The average operation time of group A was(55.36±9.883)minutes,the average hospital stay was(2.99±0.798)days,the average operation time of group B was(62.98±11.224)minutes,and the average hospital stay was(3.28±0.921)days.There were significant differences in the average operation time and the number of hospital stay between the two groups(P<0.05).6 One month after operation,SFR was 92.6%in group A and 82.5%in group B.There were statistical differences in SFR between the two groups(P<0.05).Group A was the dominant group.7 The incidence of complications was 2.5%in group A and 10.5%in group B.There were statistical differences in the incidence of complications between the two groups(P<0.05).Group A was the dominant group.Conclusions1 Compared with group B(1.5cm<diameter<2.0cm),group A(diameter<1.5cm)had higher SFR,lower complication rate and more significant curative effect,FURL is more suitable for renal calculi with diameter less than 1.5cm.2 When we discuss the surgical plan with patients whose diameter of kidney stones is more than1.5cm,we must submit these data to patients in order to make the most targeted treatment strategy.Figure0;Table9;Reference 160...
