Objective: The treatment of nasopharyngeal carcinoma(NPC)has entered the era of combining multiple treatments,but radiotherapy is still the main method of NPC treatment.The delineation of radiotherapy target area of nasopharyngeal carcinoma mainly depends on the local invasion characteristics of nasopharyngeal carcinoma.Although some related studies have proposed the characteristics of biological invasion of nasopharyngeal carcinoma,there is no specific study on the characteristics and invasion model of specific tissue invasion of nasopharyngeal carcinoma in central China.Therefore,this study mainly discusses the probability of nasopharyngeal carcinoma's invasion of surrounding tissues,summarizes the invasion rules,and on this basis,puts forward relevant Suggestions for the delineation of the target area of nasopharyngeal carcinoma.Methods: Collecting the data of 1010 patients with nasopharyngeal carcinoma diagnosed by pathology from January 2013 to October 2018.Previous medical history was also collected.The invasion probability of various related anatomical structures and regional lymph node metastasis of all patients were analyzed,and the invasion model of nasopharyngeal carcinoma was obtained.Chi-square test and other methods were used for statistical analysis of data,and P<0.05 was considered statistically significant.Results: The collected anatomical structures of patients with nasopharyngeal cancer were analyzed,and the anatomical structures around nasopharynx were defined as three risk levels: high-risk structure(invasion rate > 35%),medium-risk structure(5% < invasion rate ?35%)and low-risk structure(invasion rate 5%).The invasion of nasopharyngeal carcinoma was found to be a local invasion according to the pattern from proximal to distal,and it could invade the intracranial organization through the natural basilar nerve or vascular foramen.Cervical lymph nodes are progressive from top to bottom,and skipped metastasis is really rare.Conclusion: The invasion of local anatomical tissue of nasopharyngeal carcinoma is a pattern of invasion from proximal to distal,and the probability of invasion of middle-risk tissue is significantly increased when the high-risk anatomy have been invaded,and nasopharyngeal carcinoma can be invaded to a distance through the congenital skull base foramen.Presently,there is no standard criterion for the target area delineation of CTV,and individualized target area delineation brings convenience to the treatment of patients.Lymph node metastasis of nasopharyngeal carcinoma is from the upper neck down to the neck stage by stage,skipped metastasis is rare.Clinically,radiotherapy of cervical lymph nodes can be selected individually according to lymph node metastasis.It can not only ensure the survival of patients,but also improve the quality of life.
