Objective:To investigate the effects of overweight and obesity on cardiac structure and function in elderly patients with hypertension.Methods:Medical records of patients discharged from the First Affiliated Hospital of Chongqing Medical University between Apr.2014and Apr.2018 were collected,among which a total of 1183 patients with primary hypertension and?65 years old were selected.These elderly patients with hypertension were divided into four groups according to body mass index(BMI),including the low body weight group(n=61,BMI<18.5 kg/m~2),the normal body weight group(n=375,18.5kg/m~2?BMI<23kg/m~2),the overweight group(n=530,23kg/m~2?BMI<27.5 kg/m~2),and the obesity group(n=217,BMI?27.5 kg/m~2).Differences in cardiac ultrasound parameters and clinical data among the 4 groups were initially analyzed and then the correlation analysis between each parameter and BMI was conducted.Finally,the ordinal multifactorial regression analysis of relevant risk factors and BMI was performed.Results:BMI was positively correlated with sex,body weight,drinking rate,smoking rate,systolic blood pressure,history of hypertension,constituent ratio of type 2 diabetes,glycosylated hemoglobin,fasting plasma glucose,constituent of gout and fatty liver,triglyceride,cardiac function,aortic diameter,left ventricular posterior wall thickness,intervent-ricular septal thickness,left ventricular end systolic diameter,left ventricular end diastolic diameter,left atrial diameter,constituent ratio of patients with E/A<1,left ventricular mass,and left ventricular mass index(p<0.05),and negatively correlated with age,HDL-C,left ventricular ejection fraction and left ventricle fraction shortening(p<0.05).Ordinal multifactorial regression analysis showed that BMI was independently associated with left ventricular end diastolic diameter,left ventricular posterior wall thickness,interventricular septal thickness,left ventricular mass,and left ventricular mass index(p<0.05).Conclusion:In elderly patients with hypertension,overweight and obesity independently lead to enlarged left ventricular end diastolic diameter,increased interventricular septal thickness and left ventricular posterior wall thickness,and increased left ventricular mass and left ventricular mass index.However,overweight and obesity are not independently associated with the cardiac systolic and diastolic function.
