ObjectTo explore the differential diagnosis value of relative fractional anisotropy(rFA)value,relative apparent diffusion coefficient(rADC)value and their gradient of diffusion tensor imaging(DTI)parameters in different(near,middle and distal)peritumoral edema regions of cranial single high-grade gliomas(HGGs)and metastasis(METs).MethodsThe DTI data of 45 patients(25 cases of HGGs and 20 cases of METS)confirmed by pathology were retrospectively analyzed.In the axial section of the longest diameter edema area,we measured the value of FA and ADC in different distance regions(near,middle and far)of peritumoral edema from tumors and the value in mirror brain parenchyma region.Then,the value of each rFA(rFA1,rFA2,rFA3)and rADC(rADC1,rADC2,rADC3)were obtained by comparing the bilateral datas.Finally,the gradient of rFA and rADC were calculated by ADC2-rADC1,rADC3-rADC2,rADC3-rADC1,rFA2-rFA1,rFA3-rFA2,rFA3-rFA1,respectively named G1,G2,G3,G4,G5 and G6.We studyed the differences and the differential diagnosis value of these parameters above in METs and HGGs.ResultsThe rFA and rADC value of HGGs group were respcvtively lower than those parameters of METs group in any peritumoral edema region;Except rFA2 and rFA3,there were significantly statistical differences in rFA1、rADC1、rADC2 and rADC3 among them(P<0.05).And the gradient G1、G2、G3、G4 and G6 were statistically significant,too.The ROC analysis showed that the area under the curve of rFA1、rADC1、rADC2、rADC3、G1、G2、G3、G4 and G6 were 0.774、0.969、0.865、0.851、0.711、0.678、0.857、0.858、0.827,repectively.ConclusionThe rFA,rADC value and their gradient in the different peritumoral edema regions all have certain value of differential diagnosis of HGGs and METs.The rADC1 value in the near region is the most valuable,the corresponding cut-off is 0.209. |
