ObjectiveTo analyze the causes and preventive measures of complication intracranial hemorrhage of treatment of brain arteriovenous malformation by transvenous embolization by reviewing the literature systematically.MethodThe clinical characteristics of 111 cases of brain arteriovenous malformation treated by transvenous embolization in our hospital reported from 2010 to 2018 were discussed and summarized in combination with literatures.ResultA total of 111 patients had 112 brain arteriovenous malformation,and105 brain arteriovenous malformation(94%)had complete embolization.7cases(6.3%)had surgical-related complications,including 5 cases of intracranial hemorrhage and 2 cases of other surgical-related complications.Among them,3 cases had intraventricular hemorrhage after operation,2cases had subarachnoid hemorrhage due to rupture of drainage vein during operation,1 case had hemorrhage transformation after venous infarction around malformed vascular mass,and 1 case had mild hemiplegia after operation due to obstruction of anterior central venous return.ConclusionFor the ruptured brain arteriovenous malformation with diameter of malformed vascular mass less than 3cm,with single venous drainage and good venous vascular conditions,which is not suitable for surgery or radiotherapy,transvenous embolization can cure brain arteriovenous malformation.Superselective placement of microcatheters to avoid occlusion of drainage veins before complete embolization of malformed vascular masses can effectively prevent the occurrence of intracranial hemorrhage and reduce the rate of surgical complications related to transvenous embolization of brain arteriovenous malformation.
