Objective: to early explore the clinical characteristics in extremely premature infants(EPI)at low risk for early-onset sepsis(EOS),and avoid the excessive antibiotics therapy.Method: clinical data of extremely premature infants in the Department of Newborn from January 1,2010 to December 31,2017 were collected.Low risk infants was defined by born without rupture of membranes and maternal clinical chorioamnionitis.The comparison group were the rest who did not reach the low risk criteria.EOS was defined by the positive of pathogenic bacterial in blood or cerebral fluid culture?72h age.The clinical characteristics?manifestation?therapy and outcome were retrospectively analyzed.Results: in 245 EPI,153 infants met the low risk criteria.EOS incidence was 10 cases of EOS with the morbidity at 6.5% in the low-risk group verous 14 cases of EOS with the morbidity at 15.2% in the comparison group [odds ratio=0.408,95% confidence interval:0.17-0.981,P < 0.05].The mean total duration of antibiotics administration in the low-risk group was longer than that in the comparison group,which was significant difference(P=0.014).Among the low-risk group who receipt prolonged antibiotics,the risk of subsequent mortality and pulmonary outcomes was higher.Conclusion: delivery characteristics is useful in early recognition of low risk for EOS in EPI,and help for limiting empirical antibiotics therapy.
