Objective:The aim of this study was to explore the effects of continuous renal replacement therapy(CRRT)on renal function and prognosis in patients with acute renal injury(AKI)in intensive care unit(ICU)by detecting serum neutrophil gelatinase-associated lipocalin(NGAL).Methods:A total of 50 AKI(patientsUnified selection of AKI 2)patientswith acute renal injury due to infection,trauma and shock were collected from Chengde Central Hospital of ICU in June 2017 and April 2018.Serum NGAL and Cr were collected from patients who needed CRRT.Serum Cr and NGAL were c ollected 24 hours after starting CRRT.The 28-day survival of patients was div ided into survival group(group A)and death group(group B).NGAL was co mpared between survival group and death group,and the 28-day renal functio n outcome(recovery of renal function and loss of renal function)was tracked.The database of the subjects was established by Excel software,and the data w ere processed by SPSS21.0 statistical software.The changes of serum NGAL l evels in living and dead groups were analyzed by one-way ANOVA;the chan ges of serum NGAL and creatinine levels in AKI patients before and after CR RT were analyzed by one-way ANOVA;the comparison between groups was analyzed by the statistical method of two independent samples t test;Montiori ng NGAl after CRRT initiation to predict 28 day renal function outcome and mortalityusingROCThe truncation value is determined according to the ROC c urve,and the sensitivity and specificity are determined according to the truncation value.P < 0.05 was statistically significant.Logistic regression curve anal ysis of renal function outcome and prognostic certainty in ICU AKI patients tr eated with CRRT.Results:1.General data comparison of two groups of patients Compared with the control group,there was no significant difference in p and the control group(P < 0.05).2.After 4 weeks of treatment,24(48%)of the 50 patients survived in the near1 The comparison of the general conditions of the two groups of patients future after consultation.Of the survivors,8(18%)recovered renal function an d 16(34%)lost renal function.26 cases(52%)died.The area under the ROC c urve(AUC),sensitivity,specificity,Yoden index and 95% CI of AUC were 0.792,60%,92%,0.60 and 0.668-0.915 respectively.Cr+NGAL was 0.865,93%,64%,0.64,0.746-0.983.The mortality predicted by Cr+NGAL was 0.943,93%,94%,0.89,0.872-1.000.Conclusion:1.24 hours after the start of crrt,the detection of ngal predicted the short-term outcome of renal function in patients with aki.two?2.24 hours after the start of crrt,the detection of ngal predicted the short-term mortality of aki patients.3.At the same time,combined cr was used to predict the short-term outcome of renal function in patients with aki.4.At the same time,combined cr was used to predict the short-term mortality of aki patients.
